Diabetic kidney disease (DKD) is a form of damage resulting from diabetes and represents common, severe complication the disease. Globally, DKD leading cause chronic (CKD). occurs in both type 1 2 patients, with early clinical manifestations including microalbuminuria hypertension. As condition progresses, symptoms such as massive proteinuria, hypoalbuminemia, edema may appear. The key to treating lies controlling blood glucose pressure, reducing slowing deterioration function. Acupuncture, traditional Chinese medicine treatment, has shown some therapeutic effects on through mechanisms regulating glucose, improving renal flow microcirculation, antioxidation anti-inflammation, modulating gut microbiota.

Diabetic neuropathy is one of the severe complications diabetes, which affects quality life in a patient and increases risk amputations chronic wounds. Current therapeutic approaches are symptomatically oriented, focusing on comfort non-inflammatory aspects without addressing mechanism or molecular target disease. The present study investigates effects an ethanolic polyherbal extract from Citrullus colocynthis (Bitter Apple), Curcuma longa (Turmeric), Myristica fragrans (Nutmeg) using advanced silico analytical methods. According to findings, PHE showed presence total 39 bioactive compounds GC–MS analysis, include alcohols, fatty acids, terpenoids, esters, neolignans, phenylpropanoids, steroids. Three compounds—-4-isopropyl-1,6-dimethyl-1,2,3,4-tetrahydronaphthalene (−11.4 kcal/mol), (1S,2R)-2-(4-allyl-2,6-dimethoxyphenoxy)-1-(3,4,5-trimethoxyphenyl)-1-propanol (−9.8 kcal/mol) (S)-5-Allyl-2-((1-(3,4-dimethoxyphenyl)propan-2-yl)oxy)-1,3-dimethoxybenzene (−10.3 kcal/mol)—followed Lipinski rule binding affinity with aldol reductase. Docking experiments that compound 4-isopropyl-1,6-dimethyl-1,2,3,4-tetrahydronaphthalene has high-affinity aldose reductase, enzyme involved diabetic pathophysiology, whereas dynamics simulations show long-range persistence interaction reductase physiological conditions. Therefore, this combination herbal therapy computational/analytical techniques could be leading towards innovative, multi-targeted therapies against neuropathy. Nevertheless, further studies vivo required confirm efficacy, safety, pharmacokinetics biological systems.
