This paper provides a comprehensive view of the concept flexibility in healthcare design from discussion on strategies to achieve flexibility, ways searching strategies, results achieved, theoretical approaches, and other analyses necessary for its understanding. Healthcare services are complex environments that must be flexible meet demands caused by emergency situations. For example, COVID-19 pandemic showed lack incorporation prevented efficient adaptation space support high demand, which contamination rates these environments. Towards concept, systematic literature review method was used line with Preferred Reporting Items Systematic reviews Meta Analyzes method. Based that, is divided into three levels, respectively: (concentrated room scale), conversion (focused building layout) expansion (coupled analysis buildings land entire complex). Design requirements were structured at each level, however, more case studies needed collect quantitative data facilitate application design, scarce literature. Collection such can boost development assessment tools thus facilitating this
