Extracellular vesicles (EVs), which are secreted by various cell types, hold significant potential for cancer therapy. However, there several challenges and difficulties that limit their application in clinical settings. This review, integrates the work of our team recent advancements this research field, discusses EV-based treatment strategies to guide application. The following discussed: 1) leveraging inherent properties EVs development treatments; 2) modifying using EV engineering methods improve drug loading delivery; 3) targeting key molecules tumor-derived (TDE) synthesis inhibit production; 4) clearing TDEs from tumor microenvironment.
