Abstract Biological age (BA) is important for clinical monitoring and preventing aging-related disorders disabilities. Clinical and/or cellular biomarkers are measured integrated in years using mathematical models to display an individual’s BA. To date, there not yet a single or set of biomarker(s) technique(s) that validated as providing the BA reflects best real aging status individuals. Abstract The electrocardiogram (ECG) is the most commonly used exam for evaluation of cardiovascular diseases. Here we propose that age predicted by artificial intelligence (AI) from raw ECG (ECG-age) can be a measure health. A deep neural network trained to predict patient’s 12-lead in CODE study cohort ( n = 1,558,415 patients). On 15% hold-out split, patients with ECG-age more than 8 years greater chronological have higher mortality rate (hazard ratio (HR) 1.79, p < 0.001), whereas those smaller, lower (HR 0.78, 0.001). There are limited data on the unique cardiovascular disease (CVD), non-CVD, and mortality risks of primary prevention individuals with very high coronary artery calcium (CAC; ≥1000), especially compared rates observed in secondary populations. Our study population consisted 6814 ethnically diverse 45 to 84 years age who were free known CVD from MESA (Multi-Ethnic Study Atherosclerosis), a prospective, observational, community-based cohort.
