Objectives This study aims to examine the associations of daily step count with all-cause mortality and incident cardiovascular disease (CVD) by sedentary time levels determine if minimal optimal number steps is modified high time. Numerous studies support the association of exercise training, physical activity (PA) and sedentary behaviour (SB) with both mortality morbidity outcomes. The results across have been inconsistent, no umbrella reviews yet conducted on this topic. We an review meta-analyses observational by screening articles in PubMed/MEDLINE, EMBASE Web Science databases from inception to 30 April 2024. A sedentary lifestyle, low levels of physical activity and fitness, poor dietary patterns, psychosocial stress are strongly associated with increased morbidity mortality from atherosclerotic cardiovascular disease (ASCVD). Conversely, engaging in regular activity, maintaining optimal fitness levels, adhering to a heart-healthy pattern, effectively managing body weight, ensuring adequate sleep, implementing stress-reduction strategies, addressing risk factors reduced ASCVD. This comprehensive review synthesizes current evidence large observational studies randomized controlled trials on lifestyle as determinants ASCVD health. It also briefly reviews mechanistic insights into how such shear stress, reactive oxygen species production, chronic inflammation, platelets coagulation activation, endothelial dysfunction, sympathetic hyperactivity contribute the initiation exacerbation factors. BACKGROUND: Achievement of guideline-recommended levels physical activity (≥150 minutes moderate-to-vigorous per week) is associated with lower risk adverse cardiovascular events and represents an important public health priority. Although commonly follows a “weekend warrior” pattern, in which most concentrated 1 or 2 days rather than spread more evenly across the week (regular), effects pattern range incident diseases, including cardiometabolic conditions, are unknown. METHODS: We tested associations between incidence 678 conditions 89 573 participants (62±8 years age; 56% women) UK Biobank prospective cohort study who wore accelerometer for June 2013 December 2015.
