Abstract Background Recent research highlights the dynamics of suicide risk, resulting in a shift toward real-time methodologies, such as ecological momentary assessment (EMA), to improve risk identification. However, EMA’s reliance on active self-reporting introduces challenges, including participant burden and reduced response rates during crises. This study explores potential Screenomics—a passive digital phenotyping method that captures intensive, smartphone screenshots—to detect through text-based analysis. Method Seventy-nine participants with past-month suicidal ideation or behavior completed daily EMA prompts provided data over 28 days, approximately 7.5 million screenshots. Text from screenshots was analyzed using validated dictionary encompassing suicide-related general language. Results indicated significant associations between planning specific language patterns. Detection words related thoughts risk-related strongly correlated self-reported distinct between- within-person effects highlighting dynamic nature factors. Conclusions demonstrates feasibility leveraging text for detection, offering scalable, low-burden alternative traditional methods. Findings suggest dynamic, individualized monitoring via collection could enhance prevention efforts by enabling timely, tailored interventions. Future should refine models explore diverse populations extend generalizability this innovative approach.

Abstract Background India has the world’s largest number of suicides, but there is little research on trends in suicidal thoughts, especially for individuals with psychosis. More necessary to develop preventive interventions. Smartphone-based ecological momentary assessments (EMAs) can assess dynamic symptoms, most EMA studies are conducted higher income settings and have shorter (≤1 month) follow-up periods. Objective This study aimed examine duration onset offset ideation (SI) tertiary hospital outpatients psychosis India. Methods an exploratory, secondary analysis smartphone data nested within ongoing “Smartphone Health Assessment Relapse Prevention (SHARP)” project. Tertiary (n=50) early course schizophrenia at 2 socioculturally different sites were recruited given “mindLAMP” app monitoring mood through daily surveys. The survey matched 9-item Patient Questionnaire; ninth item was used define instance SI (score ≥1). A total 14 patients ≥1 instances who met site-specific use cutoff included. We examined between- within-person variability computed timescale “episodic” (sequences consecutive observations score Positive Negative Syndrome Scale (PANSS) changes symptoms its relationship temporality SI. Results Over approximately 11 (SD 2.1) months reporting average, 3253 surveys filled by participants (median 213, IQR 147‐256). 521 reported. Monthly showed substantial within- between-person variations. Timescale summary statistics revealed episodic patterns patients, average 5.9 episodes 4.4; range:1‐14; n=65) episode lasting 2.5 days 1.5; range:1‐5.3; n=27). There lag 59, 66, 81 between time first drop PANSS positive, negative, general psychopathology scores, respectively, last reported instance. after imputation missing 12.1 228 (average lag) scores indicated that enduring vulnerability subsequent beginning clinical improvement Conclusions Our adds much-needed evidence base measure dynamics thinking individual, more targeted Further steps highlighted such as frequency “burst” hours or minutes, inclusion both active passive markers thinking.
