Abstract Background The present study aimed to investigate the predictive ability of FRAIL-NH scale for 1-year mortality in institutionalized older Japanese patients and determine cutoff value. Methods This prospective observational used data from KITAKAWACHI study, which commenced October 2021. analysis included baseline September 2021 January 2022 follow-up 2023. In total, 227 were analysis, with those who died within 1 year categorized into Died group other Alive group. Receiver operating characteristic was examine cut-off value predicting mortality. Results Thirty-three (14.5%) year, there significantly more a high level nursing care, low body mass index, lower calf circumference, malnutrition, dysphagia. addition, 117 (51.5%) cases frailty, rate higher than that based on score (8.0 [7.0, 9.5] vs. 5.0 [2.0, 7.0]) all components, except illness, percentage frailties. Furthermore, receiver characteristics yielded an area under curve (95% confidence interval) 0.806 (0.729–0.883) 6.5 points Youden's index. sensitivity, specificity, false-positive rate, false-negative accuracy 7-point (the approximated 6.5-point cutoff) 84.8%, 70.6%, 15.2%, 29.4%, 72.7%, respectively, balanced sensitivity specificity 6- 8-point cutoffs reported previous studies, intermediate accuracy. Conclusions predicted risk adults. When homes, 7 appeared be best predictor outcomes. These findings highlight critical role early frailty screening improving patient care decision-making geriatric settings.

This indicates an urgent need identify main barriers arise when providing medical care in order improve process organizing services for people over working age. AIM To study characteristics psychological and communication (problems defining model doctor-patient relationship) Russian Federation age, with aim developing recommendations based on these findings eliminate obstacles. MATERIALS AND METHODS In this study, a questionnaire was used, consisting three blocks 89 questions. The data were analyzed using Statistica 10, Stata, R-Studio programs. Pearson’s chi-square test used compare categorical variable distributions. A generalized linear impact individual outcome variable. Mann-Whitney U Kruskal-Wallis tests analyze differences between samples abnormal RESULTS total 3,942 individuals participated survey. Among participants, most prevalent barrier engaging healthcare system fear visiting facility associated potential detecting new illnesses or worse outcomes (66.6%). Approximately half respondents indicated could serve as reason not seeking attention (52%). commonly reported included assistance understanding doctor’s prescriptions (27.7%), difficulty speech due hearing problems (18.5%), frequent use incomprehensible terminology doctors (25.7%), approaching formal manner delving deeply enough (24%) CONCLUSION research conducted allow us make managerial organizational decisions more purposeful reasonable manner, create trustworthy environment, quality doctorpatient communication, provide necessary support during

ObjectiveTo investigate longitudinal changes in symptomatic and preventive medication use among community-dwelling people with without Alzheimer disease (AD) 5 years pre- post-AD diagnosis.DesignRetrospective matched cohort study.Settings ParticipantsThe sample comprised 58,496 a geriatrician/neurologist-verified AD diagnosis 1:1 for age, sex, region to Finland.MethodsMedication dispensing data were obtained from the Finnish Prescription Register. Prevalence of was evaluated every 6 months diagnosis. Longitudinal between compared using ordinal logistic regression.ResultsDuring postdiagnosis, there differences (P < .001) = .006) AD. Over prediagnosis, prevalence medications increased both During 1 year had higher increase ≥3 (+4.4 vs +2.2%) (+6.4 +2.9%) plateaued Meanwhile, decreased (−6.0%) those AD, but (+6.1%) follow-up period, larger absolute percentage antipsychotics (+22.7 +1.8%) antidepressants (+19.1 +5.0%) than same paracetamol calcium supplement by 31.1% 20.4%, respectively, The largest decrease over postdiagnosis β-blockers (−9.8%) statins (−7.0%) AD.Conclusions ImplicationsAt point following diagnosis, population-level Medication assessments during this period appear coincide discontinuation whereas minimal observed use.
