Trimethylamine-N-oxide (TMAO) is associated with various chronic diseases. TMAO a downstream oxidative metabolite of trimethylamine (TMA) that generated by the gut microbiota from dietary choline, carnitine, and betaine. Current analytical methods predominantly target only, due to challenge efficiently extracting quantifying TMA. The present study demonstrates simple rapid UPLC–MRM–MS method for concurrent quantification TMAO, TMA, related precursors (choline, betaine, carnitines) following methanol extraction plasma derivatization using iodoacetonitrile (IACN). Pure resulted in higher extractability TMA (up two-fold) compared both pure acetonitrile methanol/acetonitrile mixtures. showed high linearity within tested range 0.0625–100 μmol/L (determination coefficient > 0.999) an intra- (n = 3) inter-day 9) precision 2–8% along average recovery above 94% all metabolites (TMAO, L-carnitine, acetyl-L-carnitine, propionyl-L-carnitine). method’s applicability was confirmed through comparison its precursor concentrations samples overnight-fasted subjects 12) without 21) metabolic syndrome.
