The Evolut Low Risk trial demonstrated that transcatheter aortic valve replacement (TAVR) was noninferior to surgery for the primary endpoint of all-cause mortality or disabling stroke at 2 years. BACKGROUND: Coronary accessibility following redo–transcatheter aortic valve replacement (redo-TAVR) is increasingly important, particularly in younger low-risk patients. This study aimed to predict coronary after simulated Sapien-3 balloon-expandable implantation within an Evolut supra-annular, self-expanding using pre-TAVR computed tomography (CT) imaging. METHODS: A total of 219 CT scans from the Low-Risk substudy were analyzed. Virtual and valves sized CT-based diameters. Two initial implant depths analyzed, 3 5 mm. was evaluated for 2 positions: outflow at node 4 5. RESULTS: With a 3-mm depth, suitable access predicted 84% patients with 4, 31% cases ( P <0.001). improved 5-mm depth: 97% 65% When comparing 3- sinus sequestration lowest (13% versus 2%; <0.001), highest (61% 32%; CONCLUSIONS: redo-TAVR relates position Evolut, native annular anatomy. quantitative analysis may provide useful information inform refine individualized preprocedural planning TAVR guide lifetime management future redo-TAVR.
