Hyaluronic acid (HA), a biodegradable, biocompatible and non-immunogenic therapeutic polymer is key component of the cartilage extracellular matrix (ECM) has been widely used to manage two major types arthritis, osteoarthritis (OA) rheumatoid arthritis (RA). OA joints are characterized by lower concentrations depolymerized (low molecular weight) HA, resulting in reduced physiological viscoelasticity, while RA, associated immune cells over-expressed with various cell surface receptors such as CD44. Due HA's inherent viscoelastic property its ability target CD44, there surge interest developing HA-based systems deliver bioactives (drugs biologics) arthritis. Considering benefits HA management potential advantages novel delivery systems, bioactive through beginning display improved outcomes over only treatment. The include enhanced uptake due receptor-mediated targeting, prolonged retention synovium, expressions proinflammatory mediators, regeneration, drug toxicity sustained release, cost-effective This review provides an underlying rationale prepare use for applications. With special emphasis given preclinical/clinical results, this article reviews bioactive-loaded particulate carriers (organic inorganic), gels, scaffolds polymer-drug conjugates that have reported treat RA. Furthermore, identifies several challenges valuable suggestions address them. Various developments, strategies described may guide formulation scientists optimize effective approach effectively.

Gout is a commonly occurring form of inflammatory arthritis caused by persistently elevated levels uric acid. Its incidence rate rises with the increases living standards and poor dietary habits, which has considerable impact on quality life patients. Although there wide assortment drugs available for management gout, effectiveness security these are limited their chemical stability insufficient targeting. Therefore, development effective nanomedicine systems to overcome problems treat gout becomes high priority. This review provides detailed introduction research progress developing advanced nanomedicines using polymers, hydrogel, nanocapsules, lipids, bionic vesicles, inorganic artificial organelles electronically controlled conveyor carriers improve therapy.

Rationale: The utilization of dissolving microneedles (MNs) facilitates the painless delivery pharmaceuticals via transdermal route. However, conventional MNs rely on passive diffusion through gradual matrix, which can impede therapeutic efficacy delivered drugs. Methods: In this study, we present development a novel degradable active platform. This platform employs sodium bicarbonate and citric acid loaded in patch as built-in motor for deeper faster intradermal payload delivery. microparticles undergo chemical reaction when contact with tissue fluid, resulting rapid formation explosive carbon dioxide bubbles. provides necessary force to break dermal barriers enhance Results: results demonstrated that possessed excellent mechanical properties, detachment characteristics, superior drug release kinetics. Furthermore, permeation behavior exhibited enhanced distribution skin-mimicking gel porcine skin compared MNs. vivo experiments employing rat model rheumatoid arthritis showed achieved Conclusions: universal effective autonomous dynamic microneedle technology is straightforward prepare ultilize, has potential improve drugs, offering significant prospects diverse range applications.

As a chronic systemic autoimmune disease, rheumatoid arthritis (RA) not only damages joints and other organs or systems throughout the body but also torments patients' physical mental health for long time, seriously affecting their quality of life. According to incomplete statistics at present, global prevalence RA is approximately 0.5-1%, number patients increasing year by year. Currently, drug therapies are usually adopted treatment RA, such as non-steroidal anti-inflammatory drugs (NSAIDs), disease-modifying antirheumatic (DMARDs), glucocorticoids/steroids, so on. However, traditional therapy has problems half-lives, cycles requiring frequent administration, lack specificity, possible adverse reactions (such gastrointestinal side effects, skin stratum corneum barrier damage, toxicity), which greatly restrict RA. In order improve limitations drug, physical, surgical treatments large related studies on have been carried out. Among them, hydrogels widely used in research due excellent biocompatibility, mechanical properties, general adaptability. For example, can be injected into synovial cavity fluid reduce wear between joints, lubricate avoid surface degradation. This article reviews applications under different functions situation carriers through delivery routes confirms outstanding potential great significance.

Abstract: Rheumatoid arthritis (RA) is the most common form of chronic inflammatory autoimmune disease characterized by synovitis, synovial proliferation, and cellular infiltration. Further, it leads to bone erosion, destruction articular cartilage, intense joint pain, swelling, a high rate disability, causing an immense load on human health. If identified early on, patient has continuous timely treatment, many patients can achieve remission. Although research in RA made considerable progress, conventional therapies are still popular treatment options for people with RA. But, hampered various drawbacks, including higher doses, low solubility permeability, poor bioavailability, level first-pass metabolism, adaptive tolerance (ATT), long-term drug use. These drawbacks result severe side effects toxicity patients. Advances polymer science application nanotechnology delivery systems have provided new possibilities developing new-generation smart (SDDSs). The shortcomings non-specific distribution uncontrollable release traditional motivated creation next-generation SDDSs. methods significantly changed course Such improve virtue their multi-functionality targeting capabilities. ultimate objective SDDSs deliver medication at optimal time precise dosage efficiency specificity targeted site (such as cells, tissues, organs), which aid adhere better therapy. This review highlights discusses along available management.

Introduction: Rheumatoid arthritis (RA) is an inflammatory immune-mediated disease that involves synovitis, cartilage destruction, and even joint damage. Traditional agents used for RA therapy remain unsatisfactory because of their low efficiency obvious adverse effects. Therefore, we here established microenvironment-responsive targeted micelles can respond to the increase in reactive oxygen species (ROS) levels improve macrophage-specific targeting loaded drugs. Methods: We prepared ROS-responsive folate-modified curcumin (TK-FA-Cur-Ms) which thioketal (TK) was as a linker modifying polyethylene glycol 5000 (PEG ) on micellar surface. When were ROS-overexpressing microenvironment, PEG hydration layer shed, ligand FA exposed, thereby enhancing cellular uptake by macrophages through active targeting. The targeting, ROS sensitivity anti-inflammatory properties assessed vitro. Collagen-induced (CIA) rats model utilized investigate expression serum factors histology change articular vivo. Results: TK-FA-Cur-Ms had particle size 90.07 ± 3.44 nm, decreased 78.87 2.41 nm after incubation with H 2 O . exhibited vitro RAW264.7 cells significantly inhibited cytokine levels. Pharmacodynamic studies have revealed prolonged drug circulation augmented cartilage-protective effects Conclusion: unique successfully may offer effective therapeutic strategy against RA.
