Abstract The primary function of the kidneys is to maintain systemic homeostasis (disruption renal structure and results in multilevel impairment body function). Kidney diseases are characterized by a chronic, progressive course may result development chronic kidney disease (CKD). Evaluation composition proteome urinary small extracellular vesicles (sEVs) as so‐called liquid biopsy promising new research direction. Knowing sEV could allow localization cellular changes specific sections nephron or interstitial tissue before fixed changes, detectable only at an advanced stage disease, occur. Research currently underway on role sEVs diagnosis monitoring many entities. Reports literature subject include: diabetic nephropathy, focal glomerulosclerosis glomerulopathies, fibrosis various etiologies. Studies pediatric patients still few, involving piloting if groups without validation studies. Here, we review addressing use for most common disorders children. We evaluate clinical utility define limitations markers present potential biopsy.

The clinical translation of messenger mRNA (mRNA)-based therapeutics requires safe and effective delivery systems. Although considerable progress has been made on the development systems, many challenges, such as dose-limiting toxicity specific to extrahepatic tissues, still remain. Cell-derived vesicles, a type endogenous membranous particle secreted from living cells, can be leveraged load during or after their biogenesis. Currently, they have received increasing interest for due natural origin, good biocompatibility, cell-specific tropism, unique ability cross physiological barriers. In this review, we provide an overview recent advances in naturally occurring platforms biomedical applications. Furthermore, future perspectives cell-derived vesicles discussed.

The prevalence of diabetes is increasing globally, and this trend predicted to continue for future decades. Research needed uncover new ways manage its co-morbidities. A significant secondary complication kidney disease, which can ultimately result in the need renal replacement therapy, via dialysis or transplantation. Diabetic disease presents a substantial burden patients, their families global healthcare services. This review highlights studies that have harnessed genomic, epigenomic functional prediction tools novel genes pathways associated with DKD are useful identification therapeutic targets biomarkers risk stratification. Telomere length regulation specific pathway gaining attention recently because association DKD. Researchers employing both observational genetics-based identify telomere-related function decline diabetes. Studies also uncovered functions beyond immediate telomere length, such as transcriptional inflammation. summarises revealed potential harness therapeutics modulate epigenetic modifications, treatment DKD, potentially slow reduce disease.
