Given the adverse effects of anticholinergic drugs and necessity for medication evaluation tools in aging population, a comprehensive scale to assess total burden Japan was developed. We examined longitudinal association between burden, quantified using Japanese Anticholinergic Drug Risk Scale, development frailty sarcopenia older adults. In this population-based cohort study, 2044 residents without long-term care needs were randomly selected from community Kashiwa, Japan. Baseline data collected 2012, with follow-ups 2013, 2014, 2016, 2018, 2021. Medications identified through interviews assessed Screening Tool Older Persons' Appropriate Prescriptions Japanese. Abstract Purpose Secondary to the risk of antipsychotic-induced acute dystonia, prophylactic use benztropine is occasionally warranted but recommended for no longer than 7 days after initiating an antipsychotic, correlating period highest dystonia risk. Despite associated increased anticholinergic burden, many clinicians continue order periods exceeding duration. We investigated reduction duration subsequent implementation truncated electronic entry orders improve prescribing within psychiatric facility. Methods Data were collected inpatients admitted between January and June 2020 who prescribed scheduled benztropine. In a quality improvement initiative implemented in April 2022, modified from 180-day 7-day duration, with postintervention data collection. The primary outcomes included change any indication hospital, percentage patients meeting predetermined “unnecessary use” criteria. analyses discharge prescriptions (either prophylaxis or other indications) pre- periods. Results 73 77 individual patients/encounters included. Following intervention, in-hospital decreased median 14 7.5 (P < 0.05), appropriate by 92.9%. 67.1% preintervention group 29.9% group. Conclusion Decreased means sentences, during inpatient admissions, appears feasible regardless dual antipsychotic first-generation use, may reduce rates written discharge.
