Annual
wellness
visits
(AWVs)
are
an
important
component
of
primary
care
as
they
provide
preventive
services
and
opportunity
to
identify
safety
health
risk
factors
for
Medicare
beneficiaries.
However,
practices
facing
unprecedented
demands
with
high
patient
volumes,
multimorbidity,
a
rapidly
growing
aging
population,
provider
(PCP)
shortages.
RN-led
models
increasingly
recognized
major
key
providing
quality
while
relieving
PCP
demands.
This
article
describes
the
implementation
model
pilot
in
urban
family
practice
increase
AWV
completion
alleviate
burden.

pandemic

all
roles
within
healthcare
but

unique
impact

the
professional
identity
(PI)

nurses.

experience
nurses’
formation
enactment

Background:
Transformational
leadership
is
recognized
