Stroke, including hemorrhagic stroke and ischemic stroke, is a common disease of the central nervous system. It characterized by high mortality disability rate closely associated with atherosclerosis, hypertension hyperglycemia, atrial fibrillation unhealthy living habits. The continuous development surgery medications has decreased patients greatly improved prognosis. At present, direction clinical treatment research gradually shifted to repair nerve function after stroke. Osteopontin (OPN) widely distributed extracellular matrix protein. Due its structural characteristics, OPN can be cut modified into terminal fragments different functions, which play roles in various pathophysiological processes, such as formation tumors, inflammation autoimmune diseases. also become potential diagnostic therapeutic marker. In order comprehensively analyze specific role relationship stroke-related diseases, following key words were used: 'Osteopontin, atherosis, neuroplasticity, neural repair'. The purpose of this study was to investigate the clinical efficacy antibiotic-loaded bone cement (ALBC) combined with Negative pressure wound therapy (NPWT) aspiration technique in treatment multidrug-resistant diabetic foot ulcers (MDRO-DFUs). A retrospective analysis data 80 patients MDROs-DFU who were used Vacuum sealing drainage (VSD) as NPWT excipient and met inclusion criteria from January 2019 2024 at our hospital. Patients divided into an experimental group a control group, 40 cases each. received conventional treatment, routine debridement, while ALBC addition plan group. Measurements blood inflammatory indicators, hemodynamic bacterial clearance time, healing hospital stay taken before after for both groups. Inflammatory indexes, Vascular endothelial growth factor(VEGF), internal diameter dorsalis pedis arteriosus groups significantly better than those improvement more obvious that group; had shorter time trauma turnover, trauma, hospitalization compared (P<0.05).

Purpose: Microinjury can trigger in situ tissue repair. Bone transport consists of continuous microinjuries/microfracture and induces bone formation angiogenesis. Tibial cortex transverse (TTT) was found to promote angiogenesis at the foot healing diabetic ulcers (DFUs). However, underlying mechanism remains largely unknown. Methods: We divided 72 Sprague-Dawley rats with DFUs into control, sham, TTT groups. Wound measurement histology were performed evaluate wound processes. Enzyme-linked immunosorbent assay, flow cytometry, immunohistochemistry, Western Blot used assess activity endothelial progenitor cells (EPCs) Ras/Raf/MEK/ERK signaling pathway. Results: accelerated healing, improved epidermal continuity, increased dermal thickness group than control sham Higher levels serum TGF-β 1, PDGF-BB, VEGF detected group. These changes parallel expression wounds frequency EPCs both marrow peripheral circulation, which implied that secreted proliferation migration wounds. The CD31 + cells, SMA-α pathway higher Conclusion: findings showed enhanced production growth factors turn activated EPC through pathway, ultimately contributing DFU healing. Based on these findings, we proposed a theory remote microinjuries repair target tissues (ie, microinjury-induced repair, MIRR). Future studies are needed validate this theory. Abstract Objective Diabetic foot ulcer (DFU) is a common and debilitating complication of diabetes that associated with an increased risk lower-limb amputation reduced life expectancy. Tibial cortex transverse transport (TTT) has become newly alternative surgical method to facilitate healing prevent lower limb amputation. Herein, we investigated the efficacy TTT in treating DFU changes serum omentin-1 irisin levels. Methods This study prospectively recruited 52 consecutive patients who were treated TTT. The follow-up was performed weekly during first 12 weeks postoperatively every 3 months until 1 year after levels vascular endothelial growth factor (VEGF), omentin-1, undergoing determined by ELISA methods on preoperative 1st day, postoperative 2nd week 4th week. Results wound rate 92.3% (48/52) at 1-year follow-up. visual analog scale (VAS) pain scores showed significant reduction ( p < 0.001). dorsal skin temperature, ankle brachial index, blood flow significantly VEGF, notably elevated compared day also higher than Conclusion could promote reduce amputation, demonstrating promising clinical benefits treatment DFU. Increased expressions proangiogenic factors including noted early stage TTT, which may provide new mechanism promoting heal.
