The content and quality of dementia guideline recommendations relevant to physiotherapy remains unknown. What is the methodological within scope practice? Systematic review clinical practice guidelines. MEDLINE, Embase, CINAHL, AgeLine, Google, databases, websites (to January 2025). Guidelines in English, screened through a three-phase process by two independent reviewers, were eligible if adults with practice. Guideline characteristics recommendation extracted. Recommendations using GRADE methodology certainty evidence strength scores synthesised. Quality was assessed AGREE II AGREE-REX. Twenty-one guidelines identified. Twelve used methodology, eight presented scores. Thirty-three met practice, categorised into ten topics: exercise (n = 11), acupuncture 1), psychosocial environmental interventions tailored activities 2), carer support 3), pain management 5), falls risk outcomes outcome measures 4), care planning setting transitions 4). Methodological varied significantly. Non-English language publications excluded. Only presenting Current partially reflect Varying inconsistent rating methods challenge interpretation implementation.
