Abstract Fatigue affects approximately 80% of people with Multiple Sclerosis (PwMS) and can impact several domains daily life. However, the neural underpinnings fatigue in MS are still not completely clear. The aim our study was to investigate spontaneous large-scale networks functioning associated PwMS using EEG microstate approach a spectral decomposition. Forty-three relapsing–remitting patients twenty-four healthy controls (HCs) were recruited. All participants underwent an administration Modified Impact scale (MFIS) 15-min resting-state high-density recording. We compared microstates subjects, fatigued (F-MS) non-fatigued (nF-MS) MS; correlations clinical behavioral scores also analyzed. Microstates analysis showed six templates across groups frequencies. found that F-MS emerged significant decrease F, salience network, broadband beta band. Moreover, B, visual increase than subjects bands. multiple linear regression high cognitive predicted by both decrease, respectively, delta band B F. On other hand, higher physical lower occurrence F current findings suggest level might be related maladaptive network.
