Despite research illustrating the cerebellum may be a critical circuit element in epilepsies, remarkably little is known about cerebellar engagement during seizures. We therefore implemented novel method for repeated imaging of awake, chronically epileptic animals. found widespread changes calcium signals behavioral seizures

There is mounting evidence that the cerebellum impacts hippocampal functioning, but impact of on interneurons remains obscure. Using miniscopes in freely behaving animals, we find optogenetic stimulation Purkinje cells alters calcium activity a large percentage CA1 interneurons. This includes both increases and decreases activity. Remarkably, this bidirectional occurs coordinated fashion, line with interneurons' functional properties. Specifically, activated by cerebellar are commonly locomotion-active, while those inhibited rest-active We additionally subsets show altered during object investigations, suggesting role processing objects space. Importantly, these neurons also modulation stimulation: more likely to be activated, rather than inhibited, decreased opposite profile. Therefore, play

Voxel-based morphometry (VBM) was applied to explore structural changes induced by repetitive transcranial magnetic stimulation (rTMS) and the relationship with clinical outcomes. Moreover, between each segmented regional gray matter (GM) volume investigated identify circuits involved in rTMS treatment process patients drug-resistant epilepsy (DRE).

Abstract Objective Anterior temporal lobe resection (ATLR) is an effective treatment for drug‐resistant epilepsy (TLE), although language deficits may occur after both left and right ATLR. Functional reorganization of the network has been observed in ipsilateral contralateral hemispheres within 12 months ATLR, but little known longer‐term plasticity effects. Our aim was to examine functions up a decade relation cognitive profiles. Methods We examined 24 TLE patients (12 [LTLE]) 10 controls across four time points: pre‐surgery, 4 months, ~9 years post‐ATLR. Participants underwent standard neuropsychological assessments (naming, phonemic, categorical fluency tests) verbal functional magnetic resonance imaging (fMRI) task. Using flexible factorial design, we analyzed longitudinal fMRI activations from post‐ATLR, relative controls, with separate analyses people hippocampal sclerosis (HS). Change profiles correlated long‐term change determine “efficiency” reorganized networks. Results LTLE had increased engagement extra‐temporal regions, better performance linked bilateral activation. Those HS exhibited more widespread activations. RTLE showed outcomes associated these areas. Both groups achieved stability over 9 years, than 50% improving. Older age, longer duration, lower pre‐operative reserve negatively affected performance. Significance Neuroplasticity continues post‐epilepsy surgery RTLE, recovery regions. This adaptive improved outcomes, challenging traditional view localized These findings emphasize need early intervention, tailored counseling, potential continued gains extended post‐ATLR rehabilitation.
