Ten years of transitional pain service research and practice: where are we and where do we go from here?

Chronic
postsurgical
pain
(CPSP)
is
a
prevalent
yet
unintended
consequence
of
surgery
with
substantial
burdens
to
the
individual
and
their
family,
healthcare
system,
society
at
large.
The
present
article
briefly
reviews
evidence
for
transitional
services
(TPSs)
that
have
arisen
in
an
effort
prevent
mange
CPSP
persistent
opioid
use,
provides
update
on
recent
novel
risk
factors
CPSP.
Available
from
one
randomized
controlled
trial
(RCT)
three
non-randomized
cohort
studies
suggests
TPS
treatment
associated
better
use
outcomes,
including
fewer
tablets
prescribed
discharge,
weaning
results,
lower
incidence
new-onset
chronic
consumption
opioids
even
later
time
points
up
1
year
after
surgery.
Another
RCT
indicates
can
be
enhanced
by
provision
perioperative
clinical
hypnosis.
While
these
preliminary
are
generally
positive,
large-scale,
RCTs
needed
provide
more
definitive
picture
whether
TPSs
effective
reducing
improving
mental
health
outcomes
short
long
term.
With
expansion
across
North
America
globally,
care
focused
transition
chronicity
has
potential
help
millions
patients.

Long-term back pain recall in Generation XXI adolescents: the role of sensitivity and pain history

Abstract
Introduction:
Adolescence
is
a
period
of
profound
cognitive
and
affective
development,
making
it
critical
for
studying
pain
memory
its
role
in
chronic
pain.
Objective:
As
this
issue
underexplored
adolescents,
we
aimed
to
quantify
the
long-term
back
recall
assess
association
with
other
pain-related
factors
experiences.
Methods:
We
analyzed
data
1,089
participants
from
Generation
XXI
birth
cohort
(Portugal),
comparing
reported
at
age
13
(Lübeck
Pain-Screening
Questionnaire)
recalled
18
(explicit
memory),
investigating
instances
forgetting/under-recalling,
over-recalling,
concordantly
recalling
presence/absence
combined
concordant
recalls
into
single
category
under-recalling
over-recalling
any
as
discordant
recall.
Parameters
such
current
pain,
family
members
recurrent
health-related
quality
life,
environmental
sensitivity,
self-perceived
sensitivity
were
analyzed.
Results:
At
18,
small
percentage
experiencing
13,
while
12%
under-recalled
it.
Concordant
was
significantly
higher
boys
(OR:
1.62;
95%
confidence
interval
[CI]:
1.20–2.19)
individuals
1.74;
CI:
1.07–2.85).
Those
less
likely
under-recall
compared
those
without
0.21;
0.05–0.91).
A
good
life
increased
likelihood
2.91;
1.11–7.67)
but
did
not
affect
over-recall.
Conclusion:
Our
results
suggest
that
history
influence
recall,
which
could
contribute
experiences
later
life.
