Oral cancer, a subtype of head and neck poses significant global health challenges owing to its late diagnosis high metastatic potential. The epithelial cell adhesion molecule (EpCAM), transmembrane glycoprotein, has emerged as critical player in cancer biology, particularly oral stem cells (CSCs). This review highlights the multifaceted roles EPCAM regulating metastasis, tumorigenicity, resistance therapy. EpCAM influences key pathways, including Wnt/β-catenin EGFR, modulating CSC self-renewal, epithelial-to-mesenchymal transition (EMT), immune evasion. Moreover, been implicated metabolic reprogramming, epigenetic regulation, crosstalk with other signaling pathways.
