Abstract Human error is often implicated in industrial accidents and frequently found to be a symptom of broader issues within the sociotechnical system. Therefore, research exploring human during maintenance activities important. Minor errors in smart contract coding on the blockchain can lead to significant and irreversible economic losses for transaction parties. Therefore, mitigating risk posed by is crucial, necessitating development of approaches enhance human reliability coding. The Cognitive Reliability Error Analysis Method (CREAM) one such approach, examining how environmental conditions affect error probability (HEP). Within CREAM, Common Performance Conditions (CPCs) influence probability. This study ranks CPCs based their importance using Bayesian Best Worst (BWM). Two methods are developed basic CREAM. In first method, experts specify control mode opinions, experts' determined according level. second an optimization problem formulated select most suitable programs, enhancing reliability. proposed model considers energy, cost, organizational budget factors identify optimal while minimizing risks costs associated with errors. A case electronics supply chain validates applicability efficacy methods. Results from method indicate opportunistic mode. contrast, shows that improving CPC levels has a more effect, shifting towards tactical reducing HEP 0.00249.

Hazard and Operability (HAZOP) is the most widely used safety evaluation method because it systematically reviews causes results of all abnormal deviations in chemical plant facilities. However, fires explosions caused by human error occur frequently. This study combined HAZOP JSA, a risk assessment technique, to determine effectiveness preventing significant industrial accidents whereby injure workers. The 96-step distillation container cleaning process was assessed identifying factors, using three different methods: HAZOP, JSA JSA. After completing severe factors. Thirty-seven additional recommendations were discovered for storage operation, which not included factors identified through work process. For risks, directly related identified. It concluded that must be high-risk processes minimize explosions.

Hazardous chemicals logistics (HCL) management improves safety and operational efficiency; however, the process faces challenges, including hazards. Trust in government is critical for shaping sustainable safe behavior of hazardous enterprises (HCLEs). However, its interaction with governance mechanisms remains unclear, a systematic categorization governmental regulatory methods lacking. To improve relationship between HCLEs government, this study employs structural equation modeling to examine effects trust on HCLEs’ behavior. categorized into cognitive affective based reinforcement theories, whereas divided motivational punitive governance. A conceptual trust–government governance–HCLEs’ model that introduces as moderating variable formulated. The results show significantly improved behavior; positively moderated impact failed. These emphasize importance trust-based partnerships governments HCLEs, well governance, building compliance improving safety. Moreover, expands our understanding interrelationships among trust, HCL industry.
