Hazard
and
Operability
(HAZOP)
is
the
most
widely
used
safety
evaluation
method
because
it
systematically
reviews
causes
results
of
all
abnormal
deviations
in
chemical
plant
facilities.
However,
fires
explosions
caused
by
human
error
occur
frequently.
This
study
combined
HAZOP
JSA,
a
risk
assessment
technique,
to
determine
effectiveness
preventing
significant
industrial
accidents
whereby
injure
workers.
The
96-step
distillation
container
cleaning
process
was
assessed
identifying
factors,
using
three
different
methods:
HAZOP,
JSA
JSA.
After
completing
severe
factors.
Thirty-seven
additional
recommendations
were
discovered
for
storage
operation,
which

through
