Introduction and importance: The interactions between the susceptibility of host some COVID-19 vaccine components happened. Viral infections, including COVID-19, are related to pathogenesis bipolar disorder (BD). These findings could trigger an initial episode. Case presentation: A 48-year-old Syrian married female, presented out-clinic Modern Medical Centre Hospital, in July 2022 for episodes sub-maniac/mania episodes, which began within 15 days after booster dose vaccination, preceded by depressive mood. Both family personal past psychiatric and/or trauma history were negatives. Initiation carbamazepine 600 mg/day showed significant improvement 3 days; months later, was decreased 200 mg/day, as there no clinical symptoms. Clinical discussion: mRNA vaccines increase levels type I interferon (INF), inflammatory cytokine that creates a signal leading expression interleukins, INF, tumor necrosis factor-alpha. pathology BD is also unclear. Genetic predisposition, immunological effects, classification BD, itself, autoimmune disease play role. Although rapid following treatment, prolonged maintenance treatment essentially mood stabilizer should be considered initiated vaccination. Rare data available about maniac infection, but our knowledge, we reported first case onset vaccination COVID-19. Conclusion: Bipolar can present previously healthy people.
