The present objective was to estimate the association between childhood blood lead concentrations (BLC) and clinical diagnoses of influenza or pneumonia among a large, low-income, underrepresented cohort children using retrospective design. Medical records were pulled from Temple University Hospital System (TUHS) located in inner-city Philadelphia, PA 2010 2020. All ≤14 years age with an available value their medical included analysis (N = 14,217). Lead exposure measured via BLC as reported through screening practices. Respiratory outcomes, pneumonia, ICD codes. Log-binomial regression models fit prevalence ratios 95 % confidence intervals for associations pneumonia. Poisson counts recurrence. Additional examined sexually dimorphic outcomes. Compared <1 μg/dL, 1-3.4 μg/dL had 172 higher risk (95 CI: 1.74, 4.59), ≥3.5 101 1.19, 3.39) diagnosis. No statistically significant findings observed influenza. Risk males compared females, stronger females. Observed indicate that underrepresented, impoverished are at high-risk these associations, indicating public health implications.
