With rapid urbanization, intensified agricultural activities, and industrialization, groundwater resources are increasingly threatened by pollution. Industrial wastewater discharge the extensive use of fertilizers in particular, have had substantial impacts on quality. This study examines 18 samples collected from main urban area Yibin City to assess hydrochemical characteristics, spatial distribution, source attribution, water quality, human health risks. Isotopic identifies atmospheric rainfall primary recharge for area, water–rock interactions limestone dissolution playing key roles shaping its chemical composition. Applying Entropy-Weighted Water Quality Index (EWQI) a comprehensive quality assessment, found that 94.44% were rated “good”, indicating relatively high overall Deterministic risk assessments indicate 72.22% non-carcinogenic risks below threshold 1, while 66.67% carcinogenic 1.00 × 10−4. Monte Carlo simulations produced similar results, reinforcing reliability assessment.
