Abstract Per‐ and polyfluoroalkyl substances (PFAS) are ubiquitous persistent organic pollutants. The neurotoxic cardiometabolic effects of PFAS well documented, leading to the hypothesis that exposure increases dementia risk. Per- and polyfluoroalkyl substances (PFAS) have been linked to effects on human lipid profiles, with several epidemiological studies reporting associations between specific PFAS blood concentrations. However, these inconsistent, most focused cross-sectional analyses limited repeated measurements. In this study, we investigated serum concentrations major classes over a 30-year period (1986-2016) up five time points. Lipids analyzed included total cholesterol (TC), low-density lipoprotein (LDL-C), high-density (HDL-C), triglycerides (TG). This study 145 participants from The Tromsø Study, Norway, who donated plasma samples three times the period. Linear mixed-effects (LME) models assessed longitudinal classes, while multiple linear regression (MLR) were used for associations. LME demonstrated positive perfluorooctanoic acid (PFOA), perfluorononanoic (PFNA), perfluorodecanoic (PFDA), perfluoroundecanoic (PFUnDA), perfluorododecanoic (PFDoDA), perfluorotridecanoic (PFTrDA) TC. Additionally, PFOA, PFDA, PFUnDA, PFDoDA, PFTrDA associated LDL-C, PFUnDA summed perfluorooctane sulfonate isomers (∑PFOS) HDL-C.
