Abstract Background Prostate cancer (PCa) incidence and mortality rates are rising. Our previous research has shown that the combination of icariin (ICA) curcumol (CUR) induced autophagy ferroptosis in PCa cells, altered lipid metabolism. We aimed to further explore effects ICA CUR on gut microbiota, metabolism, immunity PCa. To create a diagnostic tool before biopsy for patients with prostate-specific antigen (PSA) levels < 20 ng/ml to minimize prostate biopsy-related discomfort and risks. Data from 655 who underwent transperineal at the First Affiliated Hospital of Wannan Medical College July 2021 January 2023 were collected analyzed. After applying Synthetic Minority Over-sampling TEchnique class balancing on training set, multiple machine learning models constructed by using Least Absolute Shrinkage Selection Operator (LASSO) feature selection identify significant variables. The best-performing model was selected evaluated through tenfold cross-validation ensure interpretability. XGBoost best reconstructed data, resulting following ROC scores: 0.995 (0.991–0.999), 0.945 (0.885–0.997 ), 0.920 (0.868–0.972).
