Abstract Hemispherotomy is a surgical procedure that disconnects large portion of the cerebral cortex from cortical and subcortical inputs in patients with severe refractory epilepsy. Whether disconnected - inaccessible to behavioral assessment supports consciousness remains unknown. Functional MRI studies have indicated preserved resting-state networks within hemisphere, raising possibility it may represent an ‘island awareness’. However, these can also persist unconscious states, such as anesthesia deep sleep. Here we assess capacity support by exploring its electrophysiological state, before after hemispherotomy, ten awake pediatric patients. After surgery, cortex–but not contralateral cortex–entered state dominated slow oscillations (<2 Hz) resembling those observed during sleep; further, spectral exponent, previously validated marker indexing 1/f-like decay power density, assumed values typically found brain-injured anesthetized adults. When compared reference sample, exponent were compatible wakefulness but attained levels typical sleep over cortex, suggesting island awareness.

Consciousness science is marred by disparate constructs and methodologies, making it challenging to systematically compare theories. This foundational crisis casts doubts on the scientific character of field itself. Addressing it, we propose a framework for comparing consciousness theories introducing novel inter-theory classification interface, Measure Centrality Index (MCI). Recognizing its gradient distribution, MCI assesses degree importance specific empirical measure has given theory. The study of consciousness has developed well-controlled, rigorous methods for manipulating and measuring consciousness. Yet, in the process, experimental paradigms grew farther away from everyday conscious unconscious processes, which raises concern ecological validity. In this review, we suggest that field can benefit adopting a more approach, akin to other fields cognitive science. There, approach challenged some existing hypotheses, yielded stronger effects, enabled new research questions.
