Incarcerated populations experienced high rates of SARS-CoV-2 infection and death during early phases the COVID-19 pandemic. To evaluate vaccine effectiveness in carceral context, we investigated first outbreak a California state prison following widespread rollout vaccines to residents 2021. We identified cohort 733 presumed be exposed between May 14 June 22, 46.9 % (n = 344) were vaccinated, primarily with two doses mRNA-1273 332, 93.6 %). In total, 92 PCR-positive cases identified, which (14.5 %) occurred among vaccinated residents. No required hospitalization. All nine isolates collected belonged Alpha (B.1.1.7) variant. used Cox proportional hazard regression estimate for at least one dose any start outbreak. Vaccine was 86 (95 CI: 75 %-97 against PCR-confirmed infection, similar results symptomatic infection.
