Background: Healthcare-acquired infections due to multidrug-resistant organisms (MDR-HAIs) pose globally significant challenges healthcare systems, leading increased morbidity, mortality, and costs. According the World Health Organization, COVID-19 pandemic significantly impacted burden of MDR-HAIs. The aim this study was investigate dynamics epidemiology MDR-HAIs in inpatients at University Leipzig Medical Center (ULMC) before during pandemic. Methods: We compared data from with bacterial 2017 2019 (pre-COVID-19) 2021to 2023 (during COVID-19) a cross-sectional, monocentric, retrospective survey. This focused on selected (MDROs) four clinical specimens. analyzed risk factors for using logistic regression models. Results: Out 342,705 inpatients, 32,206 were diagnosed infection. prevalence 8.09% 10.79% (p < 0.001), but proportion MDROs decreased 10.14% 8.07%. proportions 59.86% 56.67%, respectively. carbapenem-resistant Klebsiella pneumoniae Enterobacter cloacae significantly. by 18% pre-COVID-19 = 0.047). Longer hospital stays both periods. children elderly COVID-19. Conclusion: These findings show that it is also important analyze epidemiological facility level order evaluate effectiveness infection control practices even unprecedented health crises like
