Abstract Background The associations between trajectories of different health conditions and cognitive impairment among older adults were unknown. Our cohort study aimed to investigate the impact various trajectories, including sleep disturbances, depressive symptoms, functional limitations, multimorbidity, on subsequent risk impairment. Methods We conducted a prospective by using eight waves national data from Health Retirement Study (HRS 2002–2018), involving 4319 aged 60 years or in USA. Sleep disturbances symptoms measured Jenkins Scale Centers for Epidemiologic Research Depression (CES-D) scale, respectively. Functional limitations assessed activities daily living (ADLs) instrumental (IADLs), Multimorbidity status was self-reporting physician-diagnosed diseases. identified 8-year at four examinations 2002 2010 latent class trajectory modeling. screened participants 27-point HRS scale 2018 across waves. calculated hazard ratios (HR) Cox proportional models. Results During 25,914 person-years, 1230 developed In fully adjusted model 3, ADLs not associated with Compared low trajectory, we found that increasing (HR = 1.39; 95% CI 1.17–1.65), IADLs 1.88; 1.43–2.46), high multimorbidity 1.48; 1.16–1.88) all posed an elevated higher urban areas 2.30; 1.65–3.21) those who smoked 2.77; 1.91–4.02) (all P interaction < 0.05). Abstract INTRODUCTION Cognitive decline has become a growing public concern, yet large‐scale exposure data identifying the contributing factors remain limited. METHODS We conducted an exposure‐wide association study involving 1142 participants and 207 exposures, using machine learning to assess relative contribution joint effects of key factors. Cluster analysis intervention simulation trials helped identify high‐risk subpopulations potential benefits targeted interventions. RESULTS In adjusted mixed models, socioeconomic status domain emerged as strongest predictor longitudinal global cognitive score ( β = 2.91, p < 0.0001, q 0.0001), while dietary also played important role in memory function. The cluster found that “unfavorable lifestyle” dominated phenotype was associated with poorest outcomes. Simulation indicated scores could improve by shifting individuals from unfavorable favorable phenotypes. DISCUSSION health requires multidomain interventions, particularly fields, necessitates collaboration between government individuals. Highlights design, which assesses broad range is used novel variables understand their contributions findings indicate most significant contributor function, diet plays largest Increasing proportion phenotypes through interventions can significantly enhance health.

BACKGROUND People with dementia and their caregivers experience significant psychological distress may be at risk of trauma when coping mechanisms are overwhelmed. Specialist helplines play a vital role by offering immediate support information, however, little is known about how call handlers account for potential in responses. This study explored the extent to which trauma-informed principles evident responses provided on UK’s only 24-hour helpline. OBJECTIVE To explore type nature calls UK helpline within staff carers people dementia. METHODS A retrospective analysis was conducted 200 anonymised call-logs from Alzheimer Scotland’s Data were analysed using framework informed (safety, trustworthiness & transparency, choice, collaboration, empowerment). also reported theme frequency strength: captured often each appeared across dataset (expressed as percentage), while strength represented researcher’s subjective assessment theme’s intensity call-log, measured Likert scale. RESULTS Most (86%) made during daytime hours ‘carer/family member/friend,’ 'emotional support' 'carer stress' being primary reasons contact. Call-log aligned several principles, ‘collaboration’ most frequent response (69.5%) 'safety' night-time (79.4%). Across all responses, 'empowerment' emerged strongest theme, characterised empathetic, non-judgmental that acknowledged caller strengths. Overall, 'collaboration' (27%) frequently observed reflecting efforts share knowledge engage callers relevant resources. Call tailored approach based caller’s identity, ‘carers/family/friends’ displaying collaboration (70.4%). Analysis individuals seeking information themselves revealed empowerment themes (73.7%). CONCLUSIONS The findings highlight helpline's critical resource emotional experiencing stress. They contribute understanding can effectively applied such interactions. Implications developing guide particularly high-risk groups family carers, explored. CLINICALTRIAL N/A

Systematic documentation of morbidity with exposure assessment in a large group lead (Pb)-exposed workers China. Using the cluster sampling method, Pb-exposed were recruited from factory Henan Province, The information and blood concentration (BLC) each worker collected. Relationships between two sets data analyzed using logistic regression restricted cubic spline (RCS) models. Among 4538 workers, there 2780 males 1758 females, median age 34 years old, BLC ranging 7.70 to 722.70 μg/l. them, 1247 (27.4%) had multiple morbidities (comorbidity). Those highest showed 1.79-fold (significant) increase comorbidities compared those lowest (95% confidence interval = 1.43, 2.25; p < 0.001). In addition, exhibited dose–response relationship (p 0.001), after adjusting for potential confounding factors. RCS curve analysis revealed nonlinear risk comorbidity (p-overall 0.001, p-nonlinear Sensitivity further confirmed developing comorbidities. Additionally, indicate that female more susceptible Our study demonstrates was prevalent dose-dependent among exposed workers.

Introduction Individuals experiencing disadvantaged socioeconomic positions (SEPs) may be at increased risk of falls during middle and older age, these impacts factors vary according to the duration, timing sequencing exposures across life course. However, there has not been a recent systematic review this evidence. This study, therefore, aims synthesise existing knowledge on association between SEP course within middle- older-aged adults. Methods analysis We systematically searched for literature in three academic databases from database inception 15 March 2024: MEDLINE (Ovid), Embase (Ovid) PsycInfo (Ovid). The search strategy combined MeSH headings terms related SEP, falls, adults observational studies. Cohort, case-control cross-sectional studies with mean or median participant age >40 years, which report least one indicator fall outcome are published peer-reviewed journals were included. No language geographic restrictions imposed. Titles abstracts screened by reviewer 20% titles also second reviewer. Two reviewers independently full texts. Data will extracted using standardised Excel template. Using modified Quality Prognosis Studies (QUIPS) tool, bias included assessed appraised Meta-analyses conducted if sufficient homogeneity permits. Otherwise, narrative synthesis results undertaken. Ethics dissemination As is literature, no ethics approval required. Findings disseminated through journal article publication, conference presentations plain-text summaries public accessibility.
