Abstract Background Social inequalities in multimorbidity may occur due to familial and/or individual factors and differ between men women. Using population-based multi-generational data, this study aimed (1) assess the roles of parental education risk (2) examine potential effect modification by sex. Methods Data were analysed from 62 060 adults aged 50+ who participated Survey Health, Ageing Retirement Europe, comprising 14 European countries. Intergenerational educational trajectories (exposure) High–High (reference), Low–High, High–Low Low–Low, corresponding parental–individual attainments. Multimorbidity (outcome) was ascertained 2013 2020 as self-reported occurrence ≥2 diagnosed chronic conditions. Inequalities quantified multimorbidity-free years lost (MFYL) ages 50 90 estimated via differences area under standardized cumulative curves. Effect sex assessed stratification. Results Low associated with higher regardless education. Compared trajectory, Low–High −0.2 MFYL (95% confidence intervals: −0.5 0.1), 3.0 (2.4–3.5), Low–Low 2.6 (2.3–2.9) MFYL. This pattern observed for both sexes, a greater magnitude not when only diseases independently healthcare-seeking behaviours examined. Conclusions Individual main contributor intergenerational among older adults. These findings support importance achieving high mitigate risk.

This study aimed to explore the moderating role of socioeconomic status (SES) in association between multimorbidity and health-related quality life (HRQOL) among cancer patients Anhui China.
