ABSTRACT Background Depression is generally perceived from the perspective of common‐cause disease model. Gaining a better understanding dynamics within individuals might contribute to more effective treatments. Methods Current exploratory longitudinal research studied associations directionality between 43 generic questionnaire Symptom Questionnaire‐48 (SQ‐48) using dynamic time warp (DTW) analyses, in which trajectories with similar time‐dependent patterns can be identified. Data were analysed first, yielding distance matrices for all trajectories, after data aggregated. Results The 148 included patients admitted treatment their clinical depression. Undirected DTW analyses three longer series but otherwise randomly chosen showed large variability among individuals. Group‐level undirected numerous significant edges symptoms, largely clustering according eight pre‐existing subscales SQ‐48. directed five outstrength: ‘hopeless’, ‘restless’, ‘down/depressed’, ‘feeling tense’ ‘no enjoyment’, meaning that change these key preceded other symptoms. Despite extensive research efforts to mechanistically understand late-onset Alzheimer's disease (LOAD) and other complex mental health disorders, curative treatments remain elusive. We emphasize the multiscale multicausality inherent LOAD, highlighting interplay between interconnected pathophysiological processes risk factors. Systems thinking methods, such as causal loop diagrams systems dynamic models, offer powerful means capture study this complexity. In recent studies, we developed validated a diagram system dynamics model using multiple longitudinal data sets, enabling simulation of personalized interventions on various modifiable factors in LOAD. Abstract Background The prevalence of depressive symptoms and cognitive decline increases with age. We investigated their temporal dynamics in individuals aged 85 older across a 5-year follow-up period. Methods Participants were selected from the Leiden 85-plus study eligible if at least three measurements available (325 599 participants). Depressive assessed baseline yearly assessments during period up to 5 years, using 15-item Geriatric Depression Scale (GDS-15). Cognitive was measured through various tests, including Mini Mental State Exam, Stroop test, Letter Digit Coding test immediate delayed recall. A novel method, dynamic time warping analysis, employed model within individuals, undirected directed time-lag analyses, ascertain whether precede group-level aggregated results or vice versa. Although the World Health Organization's (WHO's) framework on social and structural determinants of health inequities (SSDHHI) has done much to raise awareness these determinants, it does not go far enough in considerations politics power. Sudden changes are often observed in clinical practice, yet a systematic framework to understand the underlying mechanisms is still lacking. In this article, we aim bridge gap by presenting for theoretical advancement and empirical investigation of sudden trajectories. We begin introducing features three distinct types transitions drawn from complex system research: bifurcation-induced tipping (B-tipping), noise-induced (N-tipping), rate-induced (R-tipping).
