To identify barriers veterans with bipolar disorder face to accessing chronic pain management services within a Veterans Affairs (VA) health care system.Veterans (n = 15) and providers working mid-Atlantic VA system. Data were collected from August 2017-June 2018.Veteran interviews focused on their experiences treatment, including that arose when trying access services. Provider whether they address veteran patients and, if so, what considerations arise addressing in other serious mental illnesses.Veterans at least 18 years old, had confirmed diagnosis, engaged outpatient the Clinicians provided direct same VA. Interviews lasted approximately 60 min transcribed analyzed using rapid analysis protocol.Four major themes emerged provider interviews: siloed (unintegrated uncoordinated physical care), primacy (prioritization of symptoms expense symptoms), lagging expectations (unfamiliarity comprehensive evidence-based options), provider-patient communication concerns (inefficient about treatment options).Veterans co-occurring unique compromise equitable treatment. Our findings suggest educating illnesses benefit effective non-pharmacological interventions for this group may improve coordination quality reduce disparities.

ABSTRACT Introduction Veterans with chronic pain frequently report comorbid disruptions in sleep and psychological dysfunction. The purpose of this study was to investigate whether function variables mediate the sleep–pain relationship. Knowledge regarding such contributing factors can inform development optimization treatments for disturbances pain. Materials Methods In an IRB-approved, registered clinical trial, we collected objective data from U.S. military (N = 184, ages 23-81) using wrist actigraphy 7 days self-reported survey assessing quality, intensity, (depression, anxiety, post-traumatic stress disorder, catastrophizing). We investigated associations between objectively measured quality intensity. addition, parallel mediation analyses, examined mediated these associations. Results Actigraphy showed suboptimal duration (less than hours) fragmentation most participants. Self-reported poor intensity were significantly correlated. Pain catastrophizing found association Conclusions Sleep sample included insufficient sleep, fragmented perceived quality. Analyses suggest that are via catastrophizing. finding highlights potential importance Future longitudinal research is needed determine extent which reduce might also improve both outcomes.
