BRD4, member of the Bromodomain and Extraterminal (BET) protein family, is largely acknowledged in cancer for its role super-enhancers (SEs) organization oncogenes expression regulation. Inhibition BRD4 shortcuts communication between SEs target promoters with a subsequent cell-specific repression to which cells are addicted cell death. To date, this most credited mechanism action BET inhibitors, class small molecules targeting proteins currently clinical trials several settings. However, recent evidence indicates that relevance goes beyond transcription regulation identifies as keeper genome stability. Indeed, non-transcriptional controlling DNA damage checkpoint activation repair well telomere maintenance has been proposed, throwing new lights into multiple functions opening perspectives on use BETi cancer. Here we discuss current available information non-canonical, their implications biology. Integrating already known gene regulation, propose "common" model explain genomic function. Precise spatio-temporal control of gene activity is essential for organismal development, growth, and survival in a changing environment. Decisive steps regulation involve the pausing RNA polymerase II (Pol II) early elongation, controlled release paused into productive synthesis. Here we describe factors that enable events trigger Pol gene.
