Background: Neonatal seizures remain a significant cause of morbidity and mortality worldwide. The past decade has resulted in substantial progress seizure detection understanding the impact have on developing brain. Optical monitoring such as cerebral near-infrared spectroscopy (NIRS) broadband NIRS can provide non-invasive continuous real-time changes brain metabolism haemodynamics. Aim: To perform systematic review optical biomarkers to identify haemodynamics during pre-ictal, ictal, post-ictal phases neonatal seizures. Method: A search was performed eight databases. combined three broad categories: (neonates) AND (seizures) using stepwise approach following PRISMA guidance. Results: Fifteen papers described haemodynamic and/or metabolic observed with No randomised controlled trials were identified search. Studies reported various occurring Conclusion: Clear noted neonates. Further studies are necessary determine whether NIRS-based methods be used at cot-side clear pathophysiological data

Over the past several decades, near-infrared spectroscopy (NIRS) has become a popular research and clinical tool for non-invasively measuring oxygenation of biological tissues, with particular emphasis on applications to human brain. In most cases, NIRS studies are performed using continuous-wave (CW-NIRS), which can only provide information relative changes in chromophore concentrations, such as oxygenated deoxygenated hemoglobin, well estimates tissue oxygen saturation. Another type known frequency-domain (FD-NIRS) significant advantages: it directly measure optical pathlength thus quantify scattering absorption coefficients sampled tissues direct measurements absolute concentrations. This review describes current status FD-NIRS technologies, their performance, advantages, limitations compared other methods. Significant landmarks technological progress include development both benchtop portable/wearable sensitive front-end photonic components, high-frequency phase measurements. Clinical technologies discussed context needed areas improvement. The concludes by providing roadmap toward next generation fully wearable, low-cost systems.

We have developed and optimized an imaging system to study improve the detection of brain hemorrhage quantify oxygenation. Since this is intended be used for in neonates through skull opening, i.e., fontanelle, we called it, Transfontanelle Photoacoustic Imaging (TFPAI) system. The terms optical acoustic designs, thermal safety, mechanical stability. lower limit quantification TFPAI detect location its size evaluated using in-vitro ex-vivo experiments. capability measuring tissue oxygenation vasogenic edema due blood barrier disruption are demonstrated. results obtained from our experimental evaluations strongly suggest potential utility TFPAI, as a portable modality neonatal intensive care unit. Confirmation these findings in-vivo could facilitate translation promising technology clinic.

Cerebrovascular pressure autoregulation promotes stable cerebral blood flow (CBF) across a range of arterial pressures. Cerebral (CA) is developmental process that reaches maturity around term gestation and can be monitored prenatally with both Doppler ultrasound magnetic resonance imaging (MRI) techniques. Postnatally, there are key advantages limitations to assessing CA ultrasound, MRI, near-infrared spectroscopy. Here we review these CBF monitoring techniques as well their application fetal neonatal populations at risk perturbations in CBF. Specifically, discuss fetuses intrauterine growth restriction, anemia, congenital heart disease, neonates born preterm those hypoxic-ischemic encephalopathy. We conclude the insights into future directions this field an emphasis on collaborative science precision medicine approaches.

Importance Repeated transfusions in preterm neonates with anemia of prematurity replace fetal hemoglobin (HbF) adult Hb (HbA), which has a low oxygen affinity. The reduction HbF is associated higher incidence retinopathy (ROP). Objective To assess whether and HbA are differently cerebral tissue oxygenation neonates. Design, Setting, Participants This cohort study was single-center, pilot on kinetics gestational age between 24.0 weeks 27.9 who were admitted to the neonatal intensive care unit Policlinico Universitario A. Gemelli IRCCS from December 27, 2021, May 15, 2023. ancillary ongoing, double-blind, multicenter Umbilical or Adult Donor Red Blood Cells Extremely Low Gestational Age Neonates Retinopathy Prematurity (BORN) randomized clinical trial. BORN trial outcome ROP severity receive standard packed red blood cell (PRBC) obtained RBCs donors (A-RBCs) cord (CB-RBCs). According procedures at institute’s unit, patients concurrently received continuous near-infrared spectroscopy (NIRS) monitoring. not prespecified protocol. Exposure Transfusion A-RBCs CB-RBCs. Main Outcomes Measures main regional saturation (crSO 2 ) fraction extraction (cFTOE) A-RBC CB-RBC transfusions. Cerebral NIRS monitoring performed by neonatologists nurses, blinded PRBC type. conducted starting product order, during transfusion, for subsequent 24 hours after transfusion completion. mean treatment effects CB-RBCs quantified using linear mixed model repeated measures. Results Of 23 neonates, 17 (11 male [64.7%]; median birth, 25.6 [IQR, 25.3-26.1 weeks]) birth weight 840 g (IQR, 580-900 g) included study; evaluated 42 episodes, 22 20 Globally considering all posttransfusion time points, overall crSO covariate-adjusted 5.27% lower (95% CI, 1.20%-9.34%; P = .01) than that transfusions, while cFTOE 6.18% 1.66%-10.69%; .009) A-RBCs. Conclusions Relevance findings this suggest may be more delivery tissues finding explain previously observed association high risk. It also suggests use CB meet RBC needs less 28 protect overexposure oxygen.

Oxygen is the final electron acceptor in aerobic respiration, and a lack of oxygen can result bioenergetic failure cell death. Thus, administration supplemental concentrations to overcome barriers tissue delivery (e.g., heart failure, lung disease, ischemia), rescue dying cells where cellular content low. However, balance consumption relies on tightly controlled gradients compartmentalized redox potential. While therapeutic be life-saving, it disrupt growth development, impair function, induce inflammation. Newborns, premature newborns especially, have features that confer particular susceptibility hyperoxic injury due oxidative stress. In this review, we will describe unique newborn physiology antioxidant defenses, history use population its role clinical trends mitigation neonatal injury.

An improvement in ecological validity is one of the significant challenges for 21st-century neuroscience. At same time, study neurocognitive processes real-life situations requires good control all variables relevant to results. One possible solution that combines capability creating realistic experimental scenarios with adequate test environment virtual reality. Our goal was develop an integrative research workspace involving a CW-fNIRS and head-mounted-display (HMD) technology dedicated offline online cognitive experiments. We designed repeated-measures model on group BCI-naïve participants verify our assumptions. The procedure included 3D environment-adapted variant classic n-back task (2-back version). Tasks were divided into (calibration) (feedback) sessions. In both sessions, signal recorded during within-group comparisons changes oxy-Hb concentration regions interest (the dorsolateral prefrontal cortex-DLPFC middle frontal gyrus-MFG). session, translated real-time feedback. hypothesized it would be obtain significantly higher than level-of-chance threshold classification accuracy enhanced attention engagement task) vs. relaxed state conditions. Additionally, we measured participants' subjective experiences BCI terms satisfaction. results confirmed hypotheses regarding condition. accordance hypotheses, combining fNIRS HMD technologies enables effective transfer procedures controlled VR environment. This opens new possibility more ecologically valid studies training procedures.
