Abstract Suicide attempt (SA) risk is elevated in individuals with bipolar disorder (BD), and DNA methylation patterns may serve as possible biomarkers of SA. We conducted epigenome-wide association studies (EWAS) blood associated BD was measured at > 700,000 positions a discovery cohort n = 84 adults history SA (BD/SA), 79 without (BD/non-SA), 76 non-psychiatric controls (CON). EWAS revealed six differentially methylated (DMPs) seven regions (DMRs) between BD/SA BD/non-SA, multiple immune-related genes implicated. There were no significant differences when BD/non-SA each compared to CON, suggested that epigenetics differentiating from do not differentiate CON. Weighted gene co-methylation network analysis trait enrichment the vs. contrast further corroborated immune system involvement, while ontology implicated calcium signalling. In an independent replication 48 47 fold-changes cohort’s sites showed moderate correlation across cohorts agreement on direction. both cohorts, classification accuracy for among highest CpG well information clinical interviews combined, AUC 88.8% (CI 83.8-93.8%) 82.1% 73.6-90.5%) combined epigenetic-clinical predictor respectively. Our results provide novel insight role functioning also suggest integrating levels holds promise improve assessment BD.
