ABSTRACT Background Identifying predictors for disability progression is crucial managing multiple sclerosis (MS). This study aims to explore levels of and informative factors in people with MS (PwMS) using healthcare data without detailed clinical information. Methods We conducted a case–control/cohort on from Bavaria's largest health insurance organization. The dataset included records assistive devices, nursing care, sick leaves, rehabilitation, drug therapies, diagnoses individuals MS, Crohn's disease (CD), rheumatoid arthritis (RA), controls (CTR) these diseases. used generalized linear models compare service utilization between other cohorts. A gradient‐boosting algorithm identified healthcare‐related associated PwMS, defined by increased care utilization. Results PwMS ( N = 11,961) demonstrated higher than CD 21,884), RA 105,450), CTR 82,677) groups, even at young ages. Besides expected risk like age, smoking, diabetes, psychiatric disorders, the prediction revealed that specific gynecological upper tract infections, asthma, thyroiditis were less likely need care. Conclusions Leveraging allows an objective assessment can identify progression. Our approach be applied studies large cohorts adapted diseases, measures, systems. Higher resources ages unmet improved treatment management strategies adults MS.

Multiple Sclerosis (MS) has been reported to be associated with intestinal inflammation and gut dysbiosis. To elucidate the underlying biology of MS-linked inflammation, we investigated infiltration immune cells during development spontaneous experimental autoimmune encephalomyelitis (EAE) in humanized transgenic (Tg) mice expressing HLA-DR2a human T cell receptor (TCR) specific for myelin basic protein peptide (MBP87-99)/HLA-DR2a complexes. Strikingly, noted simultaneous EAE colitis, suggesting a link between diseases central nervous system (CNS) inflammation. Examination colon these revealed MBP-specific Th17 as well recruitment neutrophils. Furthermore, observed that fecal Lipocalin-2 (Lcn-2), biomarker was significantly elevated predominantly produced by gut-infiltrating We then extended our findings MS patients demonstrate their Lcn-2 levels are compared healthy donors (HDs). The elevation correlated reduced bacterial diversity increased other markers including neutrophil elastase calprotectin. Of interest, bacteria thought beneficial inflammatory bowel disease (IBD) such Anaerobutyricum, Blautia, Roseburia , were Lcn-2-high patients. also decreasing trend serum acetate (a short-chain fatty acid) HDs. decrease relative abundance Blautia massiliensis reduction This study suggests neutrophils dysbiosis level is sensitive biological indicator multiple sclerosis.

Abstract Background Observational studies have revealed that type 2 diabetes (T2D) is associated with an increased risk of peripheral artery disease (PAD). However, whether the two diseases share a genetic basis and relationship causal remain unclear. It also unclear as to these relationships differ between ethnic groups. Methods By leveraging large-scale genome-wide association study (GWAS) summary statistics T2D (European-based: N case = 21,926, control 342,747; East Asian-based: 36,614, 155,150) PAD 5673, 359,551; 3593, 208,860), we explored correlation putative in both Europeans Asians using linkage disequilibrium score regression seven Mendelian randomization (MR) models. We performed multi-trait analysis GWAS gene-based analyses reveal candidate variants genes involved shared PAD. Results observed strong ( r g ) 0.51; p -value 9.34 × 10 −15 0.46; 1.67 −12 ). The MR provided consistent evidence for effect on ethnicities (odds ratio [OR] 1.05 1.28 1.15 1.27 Asians) but not T2D. This was influenced by total cholesterol, body mass index, systolic blood pressure, or smoking initiation according multivariable analysis, overlap further employing independent European sample through polygenic regression. Multi-trait novel European-specific single nucleotide polymorphisms (rs927742 rs1734409) Gene-based consistently identified one gene ANKFY1 gene-gene interactions (e.g., STARD10 [European-specific] AP3S2 [East Asian-specific]; KCNJ11 KCNQ1 Asian-specific]) trans-ethnic PAD, reflecting common co-occurrence Asians. Crohn's disease (CD) is an inflammatory bowel (IBD) with complex clinical manifestations such as chronic diarrhea, weight loss and hematochezia. Despite the increasing incidence worldwide, cure of CD remains extremely difficult. The rapid development high-throughput sequencing technology integrated-omics analyses in recent years has provided a new means for exploring pathogenesis, mining biomarkers designing targeted personalized therapeutics CD. Host genomics epigenomics unveil heredity-related mechanisms susceptible individuals, while microbiome metabolomics map host-microbe interactions patients. Proteomics shows great potential searching promising biomarkers. Nonetheless, single omics cannot holistically connect heterogeneity pathological behavior rise multi-omics analysis integrates genetic/epigenetic profiles protein/microbial metabolite functionality, providing hope comprehensive in-depth exploration Herein, we emphasized different features applications discussed current research limitations This review will update deepen our understanding from integration broad spectra provide evidence individualized therapeutics.
