Multimodal digital health technologies aim to improve long-term adherence in stroke patients through personalized feedback and psychological monitoring. However, the interactive effects of physiological factors on rehabilitation remain unclear. This study evaluates feedback, factors, their independent synergistic optimize elderly patients. was designed as a longitudinal study, with data collected from 180 participants across two central hospitals between March September 2024. A linear mixed model (LMM) used analyze impact monitoring, mechanisms patient adherence. Data were gathered structured questionnaires, resulting final sample size 540 points. The time effect has significant positive compliance. plan completion rate (A1) increases by 1.25 (t = 34.25) 2.28 units 62.56) mid-term follow-up (T2) (T3) respectively; self-reported compliance score (A2) 1.12 31.39) 2.3 points 64.27) at T2 T3 specific activities (A3) 1.37 50.34) 2.34 86.26); number interruptions (A4) decreases 0.89 -17.31) 2.11 times -41.17) respectively. In high-quality (D2) significantly promotes (β 0.0318, t 2.08), while excessively frequent (D1) showes negative (β=-0.0914, t=-1.93 ). terms emotion (C3) 0.1572, 2.695), depressed (C1) reduces interruption behavior (β=-0.0885). interaction is not significant, indicating that influence relatively independent. demonstrates support, are essential for enhancing High-quality, relevant improves adherence, ineffective may have adverse effects. Positive emotions within promote whereas depressive hinder recovery, underscoring importance support. Although lack effects, influences merit attention optimization interventions.
