Abstract Background The October 7, 2023 terrorist attack and subsequent war in Israel have created an unprecedented mental health crisis. This commentary examines emerging data on the psychological impact of these events argues for a paradigm shift Israeli healthcare system. Main body Recent studies reveal dramatic increase PTSD, depression, anxiety among populace. These findings underscore long-lasting pervasive nature trauma. Certain populations are disproportionately affected: women, ethnic minorities (particularly Arabs, who comprise 18.1% population), those experiencing traumatic loss, displacement, or economic hardship. groups require prioritized tailored interventions. While existing outcome research provides solid foundation treating common trauma-related disorders like anxiety, system is ill-equipped to handle surge demand. Too many clinicians lack training evidence-based trauma therapies standardized assessments. To address this gap, we advocate system-wide transformation. involves widespread assessments time- limited therapies, focus precision psychiatry individual needs, implementation task-shifting task-sharing models expand access care. Conclusion strategies crucial mitigating long-term consequences 7th attacks fostering societal resilience. Failure act decisively will exacerbate crisis, placing further strain individuals, families, society as whole.
