Background/Objectives: Myocarditis is a major cause of morbidity and mortality in children can lead to long-term heart failure, dilated cardiomyopathy, the need for transplantation, or death. New systemic inflammatory indices that combine lymphocyte, neutrophil, platelet counts have been recently used as strong prognostic markers some diseases adverse outcomes neoplasms. This study aimed investigate use new early predictive patients with pediatric myocarditis. Methods: retrospectively examined between ages >1 month <18 years who were monitored our clinic diagnosis myocarditis 1 January 2022 31 December 2024. The cases divided into two groups: fulminant (requiring inotropes extracorporeal membrane oxygenation due hemodynamic disturbance) non-fulminant index values these groups (calculated first 6 h) compared, results statistically analyzed. Results: included 122 treated during period (80 boys; median age: 11 (IQR: 8–14) years). Twenty-six (21.3%) developed immune-inflammation (SII) value group was 1300 1000–1600), while this 500 350–650) (p < 0.05). response (SIRI) 2.9 2.5–3.2) 1.5 1.2–1.8) cut-off found be 1050 SII, an AUC 0.76 (95% confidence interval: 0.80–0.96; p 0.001), 1.9 SIRI, 0.64. Conclusions: SII SIRI may independently predict prognoses children. These biomarkers are easy calculate using routine blood parameters.
