Abstract: This narrative state-of-the-art review paper describes the progress in understanding and treatment of Posttraumatic Stress Disorder (PTSD). Over last four decades, scientific landscape has matured, with many interdisciplinary contributions to its diagnosis, etiology, epidemiology. Advances genetics, neurobiology, stress pathophysiology, brain imaging have made it apparent that chronic PTSD is a systemic disorder high allostatic load. The current state includes wide variety pharmacological psychotherapeutic approaches, which are evidence-based. However, myriad challenges inherent disorder, such as individual barriers good outcome, comorbidity, emotional dysregulation, suicidality, dissociation, substance use, trauma-related guilt shame, often render response suboptimal. Ketamine and its S-enantiomer esketamine are novel pharmacotherapeutic options for treatment resistant depression (TRD). There is growing evidence on the efficacy other psychiatric disorders, including posttraumatic stress disorder (PTSD). It hypothesized that psychotherapy may further potentiate effects of (es)ketamine in disorders.Repeated oral was prescribed once or twice weekly five patients suffering from TRD comorbid PTSD. Background: Exposure to a potentially morally injurious event (PMIE) has been found be associated with range of adverse mental health outcomes. However, how the psychological consequences following PMIEs compare those encountered after traumatic, but not PMIE, remain poorly understood.Objective: The aim was qualitatively explore UK military veterans' responses experiences trauma and moral injury impact such events on wellbeing.Method: Thirty male veterans who reported exposure traumatic and/or were recruited. Semi-structured qualitative interviews conducted, data analysed using thematic analysis.Results: Six described non-morally event, 15 experiencing 9 'mixed' which simultaneously traumatic. Veterans PMIE dissonance, or clash between concurrently held sets values (e.g. versus civilian values), provoked considerable distress. Veterans' cognitions differ compared could have negative implications for daily functioning. Objective Veterans can present at nongovernment (Department of Defense, Department Affairs) mental health agencies with complex symptom constellations that frequently include posttraumatic stress disorder, depression, and generalized anxiety. To date, no veteran study has validated these measures on a treatment-seeking sample veterans outside the DoD VA. Methods We used ( N = 493) to validate assess constructs (PTSD Checklist 5, Patient Health Questionnaire-9, Generalized Anxiety Disorder-7). Abstract Purpose Despite the proliferation of research into evidence based treatment for military PTSD there is little assignment criterion and still demonstrates low remission rates. Method Thirty participants in a randomized control trial comparing Prolonged Exposure (PE) Somatic Experiencing (SE) were interviewed on their experiences therapy responses assessed using descriptive phenomenological analysis approach to delineate central tenets two therapeutic approaches. Objective: To critically evaluate published and unpublished systematic reviews meta-analyses on the safety efficacy of methylenedioxymethamphetamine-assisted psychotherapy for post-traumatic stress disorder. Methods: Six bibliometric databases grey literature were searched from inception to 9 May 2024 methylenedioxymethamphetamine (MDMA)-assisted compared alone among adults with Quality assessment using AMSTAR-2 tool was conducted independently by two investigators. Results: Fourteen comprising 20 primary studies involving up 353 participants included. All included one-to-three sessions 50–125 mg MDMA-assisted (some supplemental dosage) either 25–40 MDMA or inactive placebo psychotherapy. Four deemed high quality. Meta-analyses reported substantial benefits in improving disorder symptoms (standardised mean difference, 0.8–1.3), response rate (relative risk, 1.3–3.5) remission 2.3–2.9) alone. However, that assessed certainty evidence, evidence rated as low very due risk bias, indirectness imprecision.
