Background/Objective: Patients with chronic rhinosinusitis nasal polyps (CRSwNP) have a high morbidity of anosmia, yet there are few noninvasive biomarkers to measure treatment response. Nitric oxide (NO) is found in the paranasal sinuses at 100 times higher levels than lungs and vital for antimicrobial and/or mucociliary activities vasodilatory properties. Dupilumab has been shown improve anosmia 2 weeks as measured by University Pennsylvania Smell Identification Test (UPSIT), 22-item Sinonasal Outcome (SNOT-22), Loss (LoS) scoring. We examined use NO various collection methods monitor improvement dupilumab treatment. Methods: Adults CRSwNP confirmed computer tomography or endoscopy consented receive 300 mg every two 16 weeks. Subjects polyposis despite steroids history sinus surgery were recruited. Measurements (sNO) from nostril while humming, (nNO) breath-holding, fractional exhaled nitric (FeNO) exhaling collected baseline 1, 2, 4, 8, 12, Olfactory impairment was using UPSIT, SNOT-22, LoS scoring visit. Results: Sixteen adults, mean (range) age 43 years (25‐53 years) predominantly women (12/16). Baseline sNO values 434 ppb (203‐665 ppb) significantly increased 1150 (684‐1616 (p < 0.05). Significant improvements scores weeks; weak correlation level UPSIT SNOT-22 noted. No significant changes FeNO nNO found. specifically end Conclusion: Our small pilot study revealed early after initial administration. Thus, patients without asthma, value potential be used noninvasive, objective biomarker anosmia.

The prevalence of COVID-19 has drawn increasing attention to olfactory dysfunction among researchers. Olfactory manifests in various clinical types, influenced by numerous pathogenic factors. Despite this diversity, the underlying pathogenesis remains largely elusive, contributing a lack standardized treatment approaches. However, potential regeneration neurons within nasal cavity presents promising avenue for addressing effectively. Our review aims delve into current research landscape and modalities concerning dysfunction, emphasizing etiology, pathogenesis, interventions, role stem cells regenerating nerves. Through comprehensive examination, we aim provide valuable insights understanding onset, progression, diseases.

Objective The purpose was to evaluate the relationship between peripheral eosinophilia, Japan Epidemiological Survey of Refractory Eosinophilic Chronic Rhinosinusitis (JESREC) score, and olfactory dysfunction in chronic rhinosinusitis (CRS) patients explore accuracy specific cut points JESREC score predicting dysfunction. Methods In this cross-sectional, retrospective study, function assessed by Sniffin’ Sticks 12-item test multivariate logistic regression analyses were carried out. Receiver operating characteristic curves plotted derive cutoff values for scores criterion. Results A total 354 [mean (SD) age, 50.0 (14.9) years; 41.8% women] included final analysis. prevalence 46.3%. Individuals who had more likely be male (64.6% vs. 52.6%), have eosinophilic (ECRS) (39.0% 7.9%), a longer course CRS (2.3 years 1.5 years), higher (8.5 4.5), proportions nasal polyps (78.7% 18.9%) eosinophilia (3.3% 1.4%). analysis, percentage eosinophils (1.25, 1.13–1.37), (1.31, 1.22–1.40), bilateral lesion (2.06, 1.25–3.41), (15.83, 9.23–27.16), CT shadow (2.73, 1.69–4.43), ECRS (6.86, 3.68–12.80) associated with after controlling covariates, while neutrophils not significant. addition, area under curve 0.778 value defined as 5.5. Conclusions Peripheral high significantly risk patients, special attention should paid ≥6.

Abstract Purpose of Review Neurogenesis occurring in the olfactory epithelium is critical to continuously replace neurons maintain function, but impaired during chronic type 2 and non-type inflammation upper airways. In this review, we describe neurobiology olfaction alterations rhinosinusitis with nasal polyps (type inflammation) post-viral acute (non-type inflammation), highlighting role immune response attenuating neurogenesis as a possibly mechanism for loss smell these diseases. Recent Findings Several studies have provided relevant insights into basal stem cells direct participants progression identifying functional switch away from neuro-regenerative phenotype one contributing defense, process that induces deficient replacement neurons. The interaction between system might critically underlie ongoing inflammatory airway Summary diseases, neurogenesis, lack recovery.
