Abstract Background While numerous studies have identified a strong association between academic performance and mental health, the direction of causality remains unclear. This study aims to examine whether impacts health or vice versa. Methods A multistage sampling survey was conducted among 11,659 medical students from 50 universities across country. Two mediation models were constructed for analysis: (1) Academic stress → Academic Mental health (AAM), (2) performance (AMA). Data analyzed using descriptive statistics analysis. Results The Bootstrap test showed that 95% confidence interval (CI) ranged 0.000 0.002 in AAM model 0.003 0.008 AMA model. does not mediating effect AAM, whereas AMA, serves as mediator. Although both effects by three-parameter comparison method, ratio total 1.6% 17.7% Model fit indices SEM revealed that, aside goodness-of-fit index (GFI), did meet acceptable criteria. In contrast, good most indices. Conclusion findings support model, suggesting more plausible causal pathway which significantly affects performance. insight underscores importance addressing concerns enhance students’ outcomes, offering valuable guidance educational policy student interventions.
