Benign paroxysmal positional vertigo (BPPV) is the most common peripheral vestibular disorder, characterized by brief episodes of triggered changes in head position. While idiopathic cases are frequent, post-traumatic BPPV has been increasingly recognized, particularly individuals who have experienced traumatic brain injury (TBI). TBI, ranging from mild concussions to severe trauma, a significant cause neurological morbidity and often associated with dysfunction. The pathophysiology thought involve direct mechanical disruption otolithic organs, alterations endolymph dynamics, or vascular compromise affecting inner ear structures. Compared BPPV, tend more prolonged refractory course, requiring multiple repositioning maneuvers for symptom resolution. Additionally, concurrent pathologies, such as migraine, post-concussive dizziness, central dysfunction, may complicate diagnosis treatment. Early identification appropriate management crucial reducing disability improving quality life affected patients. This review explores epidemiology, pathophysiology, clinical characteristics, treatment considerations emphasizing importance multidisciplinary approach. Understanding relationship between TBI can enhance decision-making optimize rehabilitation strategies dysfunction following trauma.

Objective To analyze the clinical characteristics of elderly patients with benign paroxysmal positional vertigo (BPPV), to provide a basis for accurate diagnosis BPPV patients. Methods A retrospective case-control study was conducted evaluate data 12,282 diagnosed who received treatment at Vertigo Clinical Diagnosis and Treatment Research Center Beijing Tiantan Hospital, affiliated Capital Medical University, between January 2020 June 2024.
