Background The search for novel antimicrobial agents in Endodontics is constant to overcoming persistent infections. Psidium guajava L. a medicinal plant little explored Endodontics. aim of this study was produce hydroethanolic extract P. and evaluate its phytochemical composition, antibiofilm action against standard clinical strains Enterococcus faecalis Candida albicans , cytotoxicity genotoxicity on human keratinocyte cultures (HaCaT cells). findings provide new insights into the potential as an alternative endodontic agent, contributing development more effective biocompatible therapeutic strategies. Methods produced using young leaves extracted absolute ethanol ultrapure water ratio (30 g:100 mL). solid soluble, total flavonoid phenols content were determined. chemical composition determined via high-performance liquid chromatography (HPLC) analysis, then free radical suppressive activity assessed by determining IC 50 value, indicating concentration required eliminate 50% radicals. Later, minimum inhibitory (MIC) microbicidal (MMC) evaluated laboratory standards institute (CLSI) guidelines (M27-S4 M7-A9). Then, MTT (3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyl tetrazolium bromide) assay. Finally, Alamar Blue assay, micronucleus assay Data analyzed ANOVA Tukey’s test or Kruskal-Wallis Dunn’s test. Results soluble solids 3.35%. Using quercetin curve, 0.130 ± 0.110 mg/mL. In addition, curve phenolic acids, 1.770 1.540 HPLC analysis revealed peaks rutin, kaempferol major flavonoids extract. demonstrated notable antioxidant activity, with value 10.39 µg/mL. MMC values ranged 1.04–8.37 at 8.37 mg/mL reducing biofilms E. C. after 10 min. Cytotoxicity that all tested concentrations exhibited relatively low toxicity toward HaCaT cells. Genotoxicity assessment indicated minimal DNA damage concentrations. Overall, most biofilm while maintaining biocompatibility cultures.
