Telemedicine approaches for patients with cirrhosis, including vulnerable populations: A narrative review

Background:
The
COVID-19
pandemic
accelerated
the
adoption
of
telemedicine
in
health
care.
However,
cirrhosis
care
remains
underexplored.
In
particular,
patients
with
alcohol
use
disorder
(AUD)
and
hepatitis
C
virus
(HCV)
may
be
overrepresented
among
vulnerable
populations,
but
have
limited
access
to
telemedicine.
Method:
We
performed
a
literature
review
on
approaches
for
as
well
AUD
HCV
or
without
cirrhosis.
Peer-reviewed
studies
involving
direct
patient-physician
interactions
were
searched
PubMed
Google
Scholar.
Keywords
used
included
cirrhosis,
AUD,
HCV,
Abstracts
screened.
Full
texts
reviewed.
Results:
Among
videoconferencing
at
satellite
sites
shortened
time
from
liver
transplant
referral
evaluation
listing.
Telephone
calls
less
effective,
especially
those
decompensated
was
being
five
times
more
likely
prescribed
medications.
Treatment
programs
telephone
demonstrated
retention
rates
above
50%.
high
(>90%)
sustained
virological
response
rates.
Across
all
approaches,
concerns
raised
audiovisual
quality,
patient
privacy,
licensing
restrictions.
Conclusion:
Videoconferencing
is

promising

Abstract
Aims
Excessive
alcohol
consumption
is
a
major
global
health
concern,
contributing
to
millions
of
deaths
annually
and
significant
proportion
cirrhosis
cases;
however,
standardized
protocols
for
early
identification
alcohol-associated
liver
disease
are
lacking.
In
this
retrospective
cohort
study,
we
aimed
understand
the
prevalence
risk
factors
associated
with
elevated
stiffness
measurement
(LSM)
in
high-risk
patients
use
disorder
(AUD)
identify
variables
longitudinal
abstinence
outcomes.
Methods
Veterans
severe
AUD
without
known
admitted
35-day
residential
substance
treatment
program
were
offered
screening,
including
Fibroscan
evaluation.
Assessment
severity
outcomes
evaluated
longitudinally
by
chart
review.
Results
Conclusions
257
veterans
treatment,
185
underwent
evaluation,
22
identified
have
LSM
concerning
compensated
advanced
chronic
disease.
Patients
more
likely
remain
abstinent
after
1
year.
About
41%
≥
10
kPa
(5%
all
screened
patients)
confirmed
on
follow-up

routine
hepatology
