Alcohol-associated liver disease poses a significant global health burden, with rising alcohol consumption and prevalence of use disorder (AUD) contributing to increased morbidity mortality. This review examines the challenges opportunities in care candidates recipients transplant (LT) AUD. Despite advancements posttransplant patient survival, risk recurrence relapse remains substantial. Several have been identified, including (1) burden alcohol-associated disease, variable practices, systemic barriers; (2) disparities mental therapy access impact on transplant; (3) definitions, underdiagnosis, stigma affecting care; (4) post-LT relapse, its factors, consequential harm. The focuses improve AUD for LT through effective biochemical monitoring, behavioral pharmacologic approaches, creating Centers Excellence care, advocating policy reforms, ensuring insurance coverage necessary services as essential steps toward improving outcomes. also highlights unmet needs, such scarcity addiction specialists, calls further research personalized treatments, digital health, value-based models optimize setting.

The Sustained Alcohol use post-Liver Transplant (SALT) and the High-Risk Relapse (HRAR) scores were developed to predict a return alcohol after liver transplant (LT) for alcohol-associated disease. A retrospective analysis of deceased donor LT from October 2018 April 2022 was performed. All patients underwent careful pre-LT psychosocial evaluation. Data on use, substance abuse, prior rehabilitation, legal issues collected. After LT, all encouraged participate in rehabilitation programs interval phosphatidylethanol testing. Patients with disease stratified by < or > 6 months sobriety before listing. Those <6 further as acute alcoholic hepatitis (AH) NIAAA criteria non-AH. primary outcome utility SALT (<5 vs. ≥5) HRAR (<3 ≥3) (+phosphatidylethanol) within 1 year LT. Of 365 86 had sobriety, 85 sobriety; 41 AH 44 In those AH, mean time abstinence 58 days, 71% failed rehabilitation. Following drinking similar (24%) compared <6-month non-AH (15%) >6-month (22%). Only 4% returned heavy drinking. accuracy both low (accuracy 61%–63%) poor sensitivity (46% 37%), specificity (67%–68%), positive predictive value (22%–26%) moderate negative (81%–83%), respectively higher values (95%) predicting Both good identifying at risk recidivism.
