Background: With the rise of virulent pathogens, transplant community faces unprecedented challenges in caring for immunocompromised patients. Effective diagnosis, treatment, and follow-up are essential, with prompt immunosuppression adjustment hospitalization decompensated patients being critical survival. Objective: Our study aimed to determine if a nurse practitioner-driven care model improves outcomes kidney recipients who contracted COVID-19. Methods: We conducted retrospective COVID-19 (N = 101) from April 1, 2020 30, 2021. Patients were classified into 2 groups: those presenting emergency department symptoms diagnosed followed by practitioners. Nurse practitioners monitored this second group daily via telephone and/or video call. They provided symptom assessment, adjustment, health counseling, emotional support. Results: identified as decompensating had admissions arranged nearest hospital or our center. In intervention (n 66), was adjusted more rapidly (4.5 days vs 7 days), leading shorter illness duration (17 26 fewer acute injuries (50.0% 68.6%), lower readmission rates (5.0% 20%), better survival (97.0% 77.1%). Conclusion: As we have preventive treatment options COVID-19, interventions close monitoring demonstrated may be key achieving including graft patient strategies can applicable instances future outbreaks pathogens help protect save lives within population.

ABSTRACT Introduction Biliary atresia (BA) is the most common indication for liver transplantation (LT) in children. We aimed to identify risk factors associated with survival young patients BA modern era. Methods performed a retrospective analysis of aged < 2 years who received their first isolated LT available data from United Network Organ Sharing database (01/2013–12/2022). Factors included multivariable Cox regression were graft type, race/ethnicity, insurance status, laboratory pediatric end‐stage disease (PELD) score, history portal vein thrombosis, and intensive care unit (ICU) status. Results 1226 recipients included, whom 501 (40.9%) deceased donor whole grafts (DDWG), 425 (34.7%) technical variants (DDTV), 300 (24.5%) living (LDLT). LDLT more likely be white ( p = 0.008) have private 0.001). Multivariable demonstrated that ICU status (hazard ratio [HR] 3.23, 95% confidence interval [95% CI]: 1.72–6.08, 0.001) DDTV vs. (HR 3.03, CI: 1.14–8.04, 0.03) an increased patient mortality. loss 1.89, 1.19–3.00, 0.007) both DDWG 3.37, 1.65–6.87, 3.47, 1.69–7.14, LDLT. Conclusion improved years; however, socioeconomic differences exist between non‐LDLT recipients. Efforts promote early equitable referral centers offering are essential improving outcomes children BA.

Black Americans experience end-stage kidney disease (ESKD) at a disproportionately higher rate than other racial and ethnic groups in the United States. Kidney transplantation provides best outcomes for patients with ESKD. However, frequently have decreased access to transplantation. This article summarizes robust literature on disparities presents practical solution this complex issue by redesigning transplant process challenged structural institutional barriers. The authors provide detailed overview of novel African American Transplant Access Program (AATAP) its pillars cultural congruency, trust, health literacy, psychosocial support. includes thorough description program's conceptualization, creating program, preliminary results, guidance establish similarly culturally congruent programs.
