Abstract The photoplethysmogram (PPG) signal is widely used in pulse oximeters and smartwatches. A fundamental step analysing the PPG detection of heartbeats. Several beat algorithms have been proposed, although it not clear which performs best. Objective: This study aimed to: (i) develop a framework with to design test detectors; (ii) assess performance detectors different use cases; (iii) investigate how their affected by patient demographics physiology. Approach: Fifteen were assessed against electrocardiogram-derived heartbeats using data from eight datasets. Performance was F 1 score, combines sensitivity positive predictive value. Main results: Eight performed well absence movement scores ≥90% on hospital wearable collected at rest. Their poorer during exercise 55%–91%; neonates than adults 84%–96% compared 98%–99% adults; atrial fibrillation (AF) 92%–97% AF 99%–100% normal sinus rhythm. Significance: Two denoted ‘MSPTD’ ‘qppg’ best, complementary characteristics. evidence can be inform choice detector algorithm. algorithms, datasets, assessment are freely available.

This review critiques the literature supporting clinical assessment and management of cardiovascular disease risk stratification with brachial-ankle pulse wave velocity (baPWV). First, we outline what baPWV actually measures-arterial stiffness both large central elastic arteries medium-sized muscular peripheral lower limb. Second, argue that is not a surrogate for carotid-femoral velocity. While measures are dependent on properties aorta, also strongly extremities. Increased lower-extremity arterial amplifies hastens reflections at level widens pressure, increases afterload, reduces coronary perfusion. Third, used an established evaluation framework to identify value as independent vascular biomarker. There sufficient evidence support (1) proof concept; (2) prospective validation; (3) incremental value; (4) utility. However, there limited or no (5) outcomes; (6) cost-effectiveness; (8) methodological consensus; (9) reference values. Fourth, address future research requirements. The majority criteria, concept, validation, value, utility values, can be supported using existing cohort datasets, whereas outcomes cost-effectiveness criteria require investigation. consensus will expert statement. Finally, finish this by providing example practice model.
