Wearable sleep-tracking technology is of growing use in the sleep and circadian fields, including for applications across other disciplines, inclusive a variety disease states. Patients increasingly present data derived from their wearable devices to providers ever-increasing availability commercial new-generation research/clinical tools has led wide adoption wearables research, which become even more relevant given discontinuation Philips Respironics Actiwatch. Standards evaluating performance have been introduced available evidence suggests that consumer-grade exceed traditional actigraphy assessing as defined by polysomnogram. However, clear limitations exist, example, misclassification wakefulness during period, problems with tracking outside main bout or nighttime artifacts, unclear translation individuals certain characteristics comorbidities. This particular relevance when person-specific factors (like skin color obesity) negatively impact sensor potential downstream augmenting already existing healthcare disparities. holds great promise our field, features distinct such measurement autonomic parameters, estimation features, integrate self-reported, objective, passively recorded health indicators. Scientists face numerous decision points barriers incorporating actigraphy, multi-sensor devices, contemporary research/clinical-grade trackers into research. Considerations include device capabilities performance, target population goals study, outputs raw aggregate data, extraction, processing, analysis. Given difficulties implementation utilization real-world research clinical settings, following State Science review requested Sleep Research Society aims address questions. What can provide? How accurate are these data? should be taken account research? These outstanding questions surrounding considerations motivated this work, outlining practical recommendations using

SignificanceSkin color affects light penetration leading to differences in its absorption and scattering properties. COVID-19 highlighted the importance of understanding interaction with different skin types, e.g., pulse oximetry (PO) unreliably determined oxygen saturation levels people from Black ethnic minority backgrounds. Furthermore, increased use other medical wearables using provide disease information photodynamic therapies treat cancers, a thorough effect has on is important for reducing healthcare disparities.AimThe aim this work perform review optical properties implication variation technologies.ApproachPublished vivo coefficients associated colors were collated their effects depth transport mean free path (TMFP) assessed.ResultsVariation among reported values significant. We show that dark are ∼6% 74% greater than 400 1000 nm spectrum. Beyond 600 nm, TMFP skin. Wearable recordings of neurophysiological signals captured from the wrist offer enormous potential for seizure monitoring. Yet, data quality remains one most challenging factors that impact reliability. We suggest a combined assessment tool evaluation multimodal wearable data. analyzed patients with epilepsy four centers. Patients wore wristbands recording accelerometry, electrodermal activity, blood volume pulse, and skin temperature. calculated completeness assessed time device was worn (on-body), modality-specific signal scores. included 37,166 h 632 in inpatient 90,776 39 outpatient setting. All modalities were affected by artifacts. Data loss higher when using streaming (up to 49% among cohorts, averaged across respective recordings) as compared onboard storage 9%). On-body scores, estimating percentage on body, consistently high cohorts (more than 80%). Signal some modalities, based established indices, at night during day. A uniformly reported index is feasible, makes study results more comparable, contributes development devices routines necessary

Abstract The photoplethysmogram (PPG) signal is widely used in pulse oximeters and smartwatches. A fundamental step analysing the PPG detection of heartbeats. Several beat algorithms have been proposed, although it not clear which performs best. Objective: This study aimed to: (i) develop a framework with to design test detectors; (ii) assess performance detectors different use cases; (iii) investigate how their affected by patient demographics physiology. Approach: Fifteen were assessed against electrocardiogram-derived heartbeats using data from eight datasets. Performance was F 1 score, combines sensitivity positive predictive value. Main results: Eight performed well absence movement scores ≥90% on hospital wearable collected at rest. Their poorer during exercise 55%–91%; neonates than adults 84%–96% compared 98%–99% adults; atrial fibrillation (AF) 92%–97% AF 99%–100% normal sinus rhythm. Significance: Two denoted ‘MSPTD’ ‘qppg’ best, complementary characteristics. evidence can be inform choice detector algorithm. algorithms, datasets, assessment are freely available.

This review critiques the literature supporting clinical assessment and management of cardiovascular disease risk stratification with brachial-ankle pulse wave velocity (baPWV). First, we outline what baPWV actually measures-arterial stiffness both large central elastic arteries medium-sized muscular peripheral lower limb. Second, argue that is not a surrogate for carotid-femoral velocity. While measures are dependent on properties aorta, also strongly extremities. Increased lower-extremity arterial amplifies hastens reflections at level widens pressure, increases afterload, reduces coronary perfusion. Third, used an established evaluation framework to identify value as independent vascular biomarker. There sufficient evidence support (1) proof concept; (2) prospective validation; (3) incremental value; (4) utility. However, there limited or no (5) outcomes; (6) cost-effectiveness; (8) methodological consensus; (9) reference values. Fourth, address future research requirements. The majority criteria, concept, validation, value, utility values, can be supported using existing cohort datasets, whereas outcomes cost-effectiveness criteria require investigation. consensus will expert statement. Finally, finish this by providing example practice model.
