ABSTRACT Background There is debate about the causes of recent birth rate decline in high‐income countries worldwide. During pandemic, concern effects on reproductive health has caused vaccine hesitancy. We investigated association SARS‐CoV‐2 vaccination and infection with involuntary childlessness. Methods Females fertility age within a prospective multicenter cohort healthcare workers (HCW) were followed since August 2020. Data baseline health, SARS‐CoV‐2‐infection, obtained regularly updated, which serum samples collected repetitively screened for anti‐nucleocapsid anti‐spike antibodies. In October 2023, participants indicated presence childlessness onset during whereas those indicating an before pandemic excluded. The SARS‐CoV‐2‐vaccination was using univariable multivariable analysis. Sensitivity analysis performed to compare reporting birthing child Results Of 798 participants, 26 (3.2%) reported starting pandemic. childless women, 73.1% (19/26) vaccinated compared 86.0% (664/772) without ( p = 0.73). by 76.9% (20/26) 72.4% (559/772) controls 0.64).
