This study investigated the proteomic landscape of exfoliation glaucoma to find potential biomarkers. The enrolled 34 patients diagnosed with either syndrome with/without or age-related cataract. Plasma proteins were analyzed through mass spectrometry and Mendelian randomization (MR) based on data from deCODE, FinnGen, Atherosclerosis Risk in Communities (ARIC), eQTLGen, UK Biobank (UKB) cohorts infer relationships. Among 2025 plasma analyzed, 130 differentially expressed group, which exhibited elevated intraocular pressure. Our proteomics suggested that infection, immune responses including intestinal network, endocrine hormones, complement coagulation cascades are involved development glaucoma. Notably, there was a significant correlation between SVEP1 (odds ratio [OR] = 1.20, 95% confidence interval [CI] 1.10 1.31, P 0.0000428), findings corroborated an independent cohort. Further analysis predicted protective role LOXL1-AS1 its regulation expression. In MR phenome-wide association studies, associated complications After multiple testing corrections, tendency for be (OR 1.14, CI 1.11 1.16, 0.0000003) type 2 diabetes 1.07, 1.05 1.08, 0.0000067). reveals high expression is risk factor glaucoma, potentially affects affected by estradiol LOXL1-AS1. However, further research needed establish causality.
